About this file

This document is an Affidavit (Attachment 7) for a Request for Proposal (RTM RFP 25-002) issued by the City of Phoenix Retirement Department for Local Board Legal Counsel Services, specifically for the City of Phoenix Fire Pension Board and the City of Phoenix Police Pension Board. The affidavit serves as a formal submission document for potential offerors, outlining key assurances and representations that must be made when submitting a proposal for legal counsel services.

The affidavit requires offerors to confirm several critical points, including their capability to successfully undertake the contract responsibilities, understanding of all solicitation documents, and acknowledgment that the proposal is valid for 180 days after the proposal deadline. Notably, the document stipulates that all proposal-related costs will be borne solely by the offeror, and the boards reserve the right to reject any and all proposals. The affidavit also requires offerors to attest that they have not engaged in any unfair competitive practices, have not communicated proposal contents to other parties prior to opening, and to the best of their knowledge, have no disqualifying criminal history.
