About this file

The document is a Request for Substitution/Exemption form from the City of Phoenix Public Transit Department for managing Disadvantaged Business Enterprise (DBE) subcontractor changes. The form is designed to be submitted to the department's Compliance Section (ptdcompliance@phoenix.gov) for approval before making any modifications to the proposed DBE utilization statement or monthly DBE utilization form. The form allows prime contractors to request exemptions such as releasing, substituting, changing the scope of work for, or reducing the amount allocated to a DBE subcontractor.

The form requires the prime contractor to provide details about the contract, project, and affected DBE subcontractors, and mandates that the contractor document their good faith efforts in finding additional DBE participation. The form includes sections for the contractor's authorized representative to sign and provide contact information, with a final section reserved for Contract Compliance Section review and approval. The purpose of this document is to ensure transparency and compliance with DBE participation requirements in city contracts, allowing for necessary adjustments while maintaining oversight of minority and disadvantaged business involvement.
