About this file
This document is an Affidavit (Attachment 7) for a Request for Proposal (RTM RFP 25-002) issued by the City of Phoenix Retirement Department for Local Board Legal Counsel Services, specifically for the City of Phoenix Fire Pension Board and the City of Phoenix Police Pension Board. The document serves as a formal assurance and acknowledgment by an offeror submitting a proposal, outlining key commitments and understanding of the solicitation process. The proposal is valid for a minimum of 180 days after the proposal deadline, and the boards reserve the right to reject any and all proposals and select the one they believe will provide the best quality development.
The affidavit includes several critical attestations by the offeror, such as having the capability to complete the proposed responsibilities, understanding all solicitation documents, and confirming that all proposal-related costs will be solely borne by the offeror. Additionally, the offeror affirms that they have not conspired to compromise the competitive nature of the solicitation, have not communicated proposal contents to other parties prior to the official opening, and to the best of their knowledge, the information provided is true and correct. The document also requires the offeror to confirm that neither the firm nor its partners, corporate officers, or managing employees have been convicted of a felony or a crime involving moral turpitude.
