About this file

This document is an Affidavit (Attachment 7) for a Request for Proposal (RTM RFP 25-002) issued by the City of Phoenix Retirement Department, specifically for Local Board Legal Counsel Services for the City of Phoenix Fire Pension Board and Police Pension Board. The affidavit serves as a comprehensive assurance document where the potential offeror affirms multiple statements regarding their proposal submission, including their capability to provide services, understanding of solicitation documents, and commitment to the proposal's terms. The document outlines that the proposal is valid for 180 days after the submission deadline, and the Boards reserve the right to reject any or all proposals based on their judgment of best quality.

The affidavit requires the offeror to confirm several critical representations, such as not having conspired to compromise the competitive nature of the solicitation process, not communicating proposal contents to other parties prior to the official opening, and attesting that to the best of their knowledge, the information provided is true and correct. Additionally, the offeror acknowledges that all costs incurred in connection with the proposal will be borne solely by them, and the Boards will not be responsible for any associated expenses. The document also includes a declaration that neither the offeror nor any partner, corporate officer, or managing employee has been convicted of a felony or a crime involving moral turpitude.
