About this file
This document is a Past Performance Questionnaire issued by the 628th Contracting Squadron at Joint Base Charleston for use in evaluating offerors competing for a Joint Base Custodial Contract. The questionnaire is designed to be completed by organizations with direct experience with potential contractors to provide the government with performance feedback for source selection purposes.
The underlying custodial contract covers Joint Base Charleston, which consists of geographically separated facilities: the Air Force Base in North Charleston, SC and the Naval Weapons Station in Goose Creek, SC. The requirement encompasses 237 facilities totaling 1.5 million square feet and includes four cleaning level standards (Level 1 Prestige, Level 2 Basic, Level 3 Academic, and Level 4 Special Requirements). Contractor responsibilities include providing all personnel, equipment (including contractor-owned vehicles), supplies, materials, and supervision necessary to perform custodial services. The questionnaire addresses scope items such as support for multiple cleaning levels, provision of cleaning products with Safety Data Sheets, restroom supplies and dispenser maintenance, equipment provision, handling of desk-side trash receptacles, and services to restore restrooms following sewer backups. Respondents must return completed questionnaires by 27 May 2026 to stephanie.woods.7@us.af.mil. The questionnaire evaluates contractor performance across multiple criteria including management capability, communication and cooperation, records maintenance, equipment and supplies provision, personnel management, quality control procedures, environmental compliance, schedule adherence, contract responsiveness, and cost proposal accuracy. Respondents provide ratings on a scale from Exceptional to Unsatisfactory with supporting documentation required for each assessment.

Section L Attachment: Past Performance Questionnaire
The 628th Contracting Squadron, Joint Base Charleston, is in the process of competitively selecting a source for a Joint Base Custodial Contract. The contractor will provide all personnel and equipment, to include contractor owned vehicles, supplies, materials, and supervision necessary to support custodial services for Joint Base Charleston. Joint Base Charleston is geographically separated with the Air Force Base located in North Charleston, SC and the Naval Weapons Station, located in Goose Creek, SC. This requirement comprises multiple cleaning level standards (Level 1 Prestige, Level 2 Basic, Level 3 Academic, and Level 4 Special Requirements) and includes 237 facilities with 1.5M SF.
One of the considerations in proposal evaluation is the verification of the offeror’s past performance on contracts or other work efforts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.
Our areas of interest regarding the offeror are summarized in the enclosed questionnaire. In order to meet the acquisition milestones, we request your written response no later than 27 May 2026.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out electronically and e-mailed or by hand and faxed to the Point of Contact(s) identified below.
Your completed questionnaire will become part of the official Source Selection records.
