About this file
This document is a Past Performance Questionnaire (PPQ) for a Department of State solicitation (RFP #19AQMM25R0201) seeking support for the Haiti National Police (HNP). The solicitation requires a contractor to provide Subject Matter Expert (SME) Level Advisors, logistics support for HNP School and Academy Facilities, and special projects that advance the contract's objectives. Additionally, the contractor must deliver life and mission support services to the in-country Program Management Office (PMO).
The PPQ is a detailed form designed to capture past performance information from potential offerors, with sections to be completed by both the offeror and an external assessor. The form evaluates performance across multiple dimensions including quality of services, schedule compliance, cost control, business relations, and management capabilities. Assessors are asked to provide ratings ranging from Exceptional to Unsatisfactory across these categories, with the goal of helping the Department of State assess the contractor's capability to successfully execute the Haiti-focused contract.
