About this file

This Technical Evaluation Form (SAD-AF2) is a comprehensive solicitation document for maintenance services for petroleum facilities and Defense Logistics Agency (DLA) capitalized facilities. The document requires offerors to provide detailed technical responses across 12 sections, including proposed contract staffing, safety protocols, facility maintenance plans, preventative and corrective maintenance strategies, and specific service order details for two identified deficiencies.

Key requirements include performing preventative and corrective maintenance on petroleum facilities, complying with Unified Facilities Criteria (UFC), meeting safety standards (EM 385 1-1), developing facility maintenance plans, managing records, and executing service orders. The evaluation focuses on the contractor's ability to manage maintenance activities, safety performance, team composition, and approach to identifying and resolving facility deficiencies. Two specific deficiencies are highlighted: repairing leaking carbon steel piping at Facility 6543 and replacing damaged power conductors at Facility 9876. Offerors must provide detailed narratives demonstrating their technical capabilities and understanding of the Performance Work Statement (PWS) requirements.

Deficiency #1: D-DBE-006 – At Facility 6543, the 4-inch carbon steel piping feeding the Filter Separator #1 exhibits a significant leak at a threaded joint. The leak is located downstream of the Pre- Filter. Remove the leaking section of piping and replace it with a new section of 4-inch carbon steel piping utilizing flanged connections. Install new flanges on the existing pipe ends to mate with the new piping spool. Use new gaskets and flange bolts suitable for fuel service. Pressure test the repaired section of piping to verify leak tightness prior to returning the Filter Separator to service.

Deficiency #2: D-PAT-013 – At Facility 9876, within the Valve Pit associated with Tank #5, the 120V, Single-Phase Power Conductors supplying the Sump Pump Motor are damaged due to water intrusion caused by a severely corroded and breached conduit. Remove and replace the entire run of damaged 120V, Single-Phase Power Conductors from the local disconnect to the Sump Pump Motor junction box with new conductors of the same gauge (e.g., #12 AWG), insulation type (THHN/THWN), and specification, installed within a new, liquid-tight flexible metal conduit (LFMC). Ensure proper grounding, sealing, and watertight connections at all terminations to prevent future water intrusion.

Verify proper Sump Pump Motor operation after conductor replacement.
