SUBJECT: Request for Past Performance Questionnaire

We are currently participating in a proposal in response to the Government’s Request for Proposal No. FA5000-20-R-0016 for a firm fixed price contract for Boiler Maintenance at Joint Base Elmendorf-Richardson, Alaska.

The Government will be evaluating contractors for this service based on several selection factors.

One of the key factors in the Government’s final decision is past performance. To assist the Government in its assessment of our past performance, we are required to identify previous contracts and clients who are familiar with our past performance. For the purpose of this questionnaire, past performance is defined as contracts performed and/or being performed for any customer within the last three (3) years from the issuance date of the solicitation.

As one of our customers for past contracting work on _________________________, we are requesting that you complete the attached questionnaire regarding our past performance. This letter authorizes you to discuss our past performance with the Government and to release information regarding our past performance as directed.

Your prompt attention in completing the attached questionnaire will ensure we receive full consideration in this competition. Please return the completed form to the Government’s point of contacts identified below, to arrive no later than 4:30 P.M. local Alaska Time on Friday, April 4, 2020.

Exceptional Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

QUALITY OF SERVICE

Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
