EXECUTIVE SUMMARY
The proposed installation of a water disinfection system requires the Mann-Grandstaff Veteran
Affairs Medical Center to create a separate Water District to comply with Washington Department of Health cross-connection requirements. Cascade Earth Sciences was contracted by William
Wilson Architects, PC to provide cross-connection control inspection services in order to comply with Department of Health and Environmental Protection Agency requirements. This report describes cross-connection and building containment hazards that require corrective action to meet the requirements of the Uniform Plumbing Code as amended for the State of Washington.
Prioritized corrective actions and cost estimates are provided for these identified cross-connection hazards.
Cascade Earth Sciences has identified several locations throughout the Mann-Grandstaff Veteran
Affairs Medical Center requiring backflow prevention assemblies or indirect connections to prevent backflow into the domestic potable water system. Other deficiencies, such as dead end lines, were also identified as part of the inventory assessment. The majority of cross-connection issues in the
Mann-Grandstaff Veteran Affairs Medical Center are associated with water-use equipment.
This report summarizes cross-connections, dead end lines, and isolation assembly hazards identified during the facility assessment and provides a prioritized list of cross-connection hazards for correction. Locations throughout the domestic water system requiring backflow prevention assemblies or indirect connections to prevent backflow are identified. Dual check valves, double check valves, or reduced pressure backflow assemblies are required for each service connection on campus buildings to correct identified cross-connection deficiencies. Cost estimates for the proposed corrective actions are provided for reference and planning purposes.

1.0 INTRODUCTION
The project location is the Mann-Grandstaff Veteran Affairs Medical Center (MGVAMC) campus located at 4815 N Assembly Street, Spokane, Washington (Site) consisting of over 30 separate buildings. The proposed installation of a water disinfection system requires MGVAMC to create a separate Water District to comply with Washington Department of Health (DOH) Cross-Connection
Control requirements. Cascade Earth Sciences (CES) was contracted by William Wilson Architects, PC (WWA) to provide cross-connection control inspection services in order to comply with DOH and Environmental Protection Agency (EPA) requirements. This report contains a description of identified cross-connections and building containment deficiencies, a prioritized list of cross-connection hazards, and cost estimates associated with required corrective actions. Specifically, this report has been prepared to comply with the following:
• Washington Administrative Code (WAC) 51-56-003.
• U.S. Department of Veterans Affairs Office of Construction and Facilities Management
Plumbing Design Manual (PG 18-10).
• Washington State Department of Health (DOH) Cross-Connection Control for Small Water
Systems (DOH 331-234).
2.0 BUILDING CONTAINMENT DEFICIENCIES
Dual check valves, double check valves (DC), or reduced pressure backflow assemblies (RPBA) are required for each service connection to meet the containment requirements of the Uniform
Plumbing Code as amended for the State of Washington. Service lines 1-inch or smaller require a dual check valve assembly. One-story non-healthcare related service lines larger than 1-inch require a DC valve assembly. Service lines associated with hospital facilities or buildings greater than two stories require RPBA(s). Individual building containment requirements are presented in Table 1. As shown, 13 buildings on the MGVAMC campus require additional backflow protection assemblies.
Building 1 is required to have RPBA(s) installed on both the main service connections to meet cross-connection code requirements, as the building is a hospital greater than two stories high. The existing utility rooms and crawl spaces east of rooms C005 and A004 are locations where RPBA(s) could be installed for the Building 1 main service connections. Two parallel RPBA(s) for Building 1 main services will ensure the hospital has water from both connections at all times.
Buildings 25, 26, and 27 are currently connected to the Building 1 domestic water system through
Room C005D in the basement. The domestic water service for Buildings 25, 26, and 27 currently services rooms in Building 1 prior to servicing the aforementioned buildings. Buildings 26 and 27 have separate hot water heaters. Currently, the hot water line for Building 25 is connected to the primary hospital hot water system.
Buildings 25, 26, and 27 should be independently connected to the main service water line with separate connections and RPBA(s). The RPBA(s) should be installed on each service line for
Buildings 25, 26, and 27 in Room C005D. A hot water heater should be installed in Room C005D
Doc: 2018230008 Corrective Action Rpt sjr.docx September 2018 | Page 2 to make the Building 25 hot water system independent so that it is separate from the hospital system just as Buildings 26 and 27 are independent.
Expansion tanks are required when installing dual check valves or any RPBA(s) connected to water mains with hot water heaters or makeup water connections. Catastrophic failure can occur with water heaters due to water expansion if expansion tanks are not installed.
3.0 CROSS-CONNECTION HAZARDS
A list of prioritized cross-connection hazards requiring corrective action is presented in Table 2. The primary cross-connection hazard categories discovered during the assessment were water main supplies, fire protection systems, janitor’s closets, equipment, irrigation connections, and hose bibs.
Identified cross-connection hazards are grouped by category and sorted according to the level of threat imposed to the facility and Site. Table 2 provides a list of prioritized cross-connection hazards that require correction. Each hazard listed in Table 2 has a corresponding hazard assessment field sheet that can be referenced in the CES Cross-Connection Inventory Report (CES, 2018).
Example Cross-Connection Control Hazard Assessment identification code: 1-001-C406-001
Where:
1 = Building
004 = Floor
C406 = Room Number
001 = Cross Connection ID number
One potential direct cross-connection is located in Building 1. The domestic cold water line in
Hallway C4-B1 outside of Room A429 on the 4th Floor was unable to be tracked beyond that location and could potentially connect to non-domestic plumbing without an RPBA. It remains undetermined if this cold domestic line poses a direct cross-connection risk requiring a RPBA.
Further investigation is required to determine the destination of this cold domestic line and potential cross-connection risk.
4.0 OTHER CONCERNS
Dead end lines are potential sources of stagnation and can lead to contamination of the domestic water system. Twenty dead end lines were identified during the Site assessment and are listed along with locations in Table 3. Dead end lines require corrective action and should be removed and capped next to the branch line or at the stack.
5.0 COST ESTIMATES
Cost estimates are divided into the following categories: main service connections, fire protection systems, hot water makeup water, janitor’s closets, hose bibs, and equipment. These estimates are provided in Table 4.
Doc: 2018230008 Corrective Action Rpt sjr.docx September 2018 | Page 3
The appropriate cross-connection prevention valve assemblies are available in a wide range of costs.
Price quotes of median priced, high quality Watts brand units are used for cost estimation purposes.
Assemblies of 4-inch and larger have assumed costs for fittings and supplies of 80% of the assembly cost. Fittings and supplies of assemblies smaller than 4-inch are estimated at 40% of the assembly cost. The labor costs are assumed at 100% of the cost of the combined assembly, fittings, and supplies. Total costs of installed assemblies will vary depending on the location and complexity of individual cross-connection hazards.
Two cost estimate options are provided for the janitor’s closet soap dispensers. The soap dispenser units meet cross-connection requirements due to the built in air gap. The cross-connection risk arises when soap dispenser units are connected to the janitor’s sink while the domestic water is left
on. This creates a direct cross-connection between the domestic hot and cold water. If the plumbing to these sinks is modified so only a single water source is used, either hot or cold, then no cross-connection prevention assembly is required. If mixed hot and cold water is required prior to discharge, then two assemblies will be needed, one for each domestic line. These two options are included in the Table 4 cost estimates.
6.0 CONCLUSIONS
CES has identified cross-connection and building containment hazards throughout the Site that require corrective action to meet the requirements of the Uniform Plumbing Code as amended for the State of Washington. The majority of cross-connection issues in the Site are associated with equipment. Lack of RPBA(s) make up the majority of these issues. Prioritized corrective actions and cost estimates are provided for these identified cross-connection and building containment hazards.
This Corrective Action Report is to be used in conjunction with the accompanying CES Cross-
Connection Inventory Report and Cross-Connection Control Plan to address cross-connection control deficiencies across the Site. As identified cross-connection deficiencies are corrected throughout the Site, the Cross-Connection Inventory should be updated, creating an ongoing record for management to track cross-connection corrections.
