EXECUTIVE SUMMARY
The proposed installation of a water disinfection system requires the Mann-Grandstaff Veteran
Affairs Medical Center to create a separate Water District to comply with Washington Department of Health cross-connection requirements. Cascade Earth Sciences was contracted by William
Wilson Architects, PC to provide services in order to comply with Department of Health and
Environmental Protection Agency requirements associated with cross-connection control deficiencies. This report contains a description of the Mann-Grandstaff Veteran Affairs Medical
Center Campus buildings and provides a baseline inventory of the domestic water plumbing services and existing potential cross-connections.
The facility survey methodology is discussed in Section 2. Locations throughout the domestic water system requiring backflow prevention assemblies or indirect connections to prevent backflow of contaminants are identified. Additional deficiencies such as dead end lines and required check valves are noted.
An inventory of existing cross-connections and domestic plumbing services is discussed in
Section 3. The inventory consists of surveyed connections to the domestic water system including rooms with equipment connections and all associated plumbing service connections. The domestic plumbing system was analyzed and inventoried separate from equipment connections and compared to existing engineering records.
The majority of cross-connection issues in the Mann-Grandstaff Veterans Affairs Medical Center are associated with equipment. Lack of backflow assemblies account for the majority of issues. The primary cross-connection categories are: water main supplies, fire protection systems, janitor closets, equipment, irrigation connections, and hose bibs.

1.0 INTRODUCTION
The project location is the Mann-Grandstaff Veteran Affairs Medical Center (MGVAMC) campus located at 4815 N Assembly Street, Spokane, Washington (Site) consisting of over 30 separate buildings. Appendix A provides a reference of campus buildings. The proposed installation of a water disinfection system requires MGVAMC to create a separate Water District to comply with
Washington Department of Health (DOH) cross-connection control requirements. Cascade Earth
Sciences (CES) was contracted by William Wilson Architects, PC (WWA) to provide services in order to comply with DOH and Environmental Protection Agency (EPA) requirements associated with cross-connection control deficiencies. This report contains a description of the CES Site assessment and provides an inventory of the domestic water plumbing and potential cross-connections.
2.0 FACILITY SURVEY
CES conducted a visual survey (survey) of the water supply connections within the MGVAMC.
The buildings surveyed are listed in Table 1. The Survey included buildings 1, 2, 3, 12, 14, 25, 26, 27, 30, 37, 40, and 41. Buildings 14A, 16, 38, and 39 had no domestic water service. Portions of buildings 4, 5, 6, 6A, 7, 31, 32, and 33 were not mapped due to the location of domestic plumbing lines in confined spaces. The survey was limited to a walk-through visual inspection of all fittings and connections accessible through access ports and doors. CES identified all sources of backflow, air gaps, or other potential contamination based on these connections. Connections that required physical intrusion of walls, floors, ceilings, confined spaces, or other building structures were outside the scope of this project, and therefore not surveyed.
When required, CES personnel were accompanied by Greg Baggarley or Jeremy Kasprak of the
MGVAMC Facility Maintenance Department. Mr. Baggarley and Mr. Kasprak were relied upon, as needed, to direct CES personnel to equipment locations, drain locations, cross-connections between domestic water and non-potable systems, and fire and domestic water building service entrances.
Site main lines were surveyed, where possible, for comparison to engineering records provided by
MGVAMC. Figure 1 provides the location of current Site main and irrigation lines. Domestic water plumbing was surveyed within each building and compared to the existing plan drawings provided by MGVAMC. Appendix B provides plumbing survey field notes of traced domestic cold, hot, hot recirculation, and drinking water lines throughout the Site. Stack connections were located and plumbing services were traced for each floor and room connection when possible. Each stack service was evaluated for plumbing cross-connections involving the domestic hot, recirculation, and cold water systems.
3.0 INVENTORY
A baseline inventory was created of surveyed connections to the domestic water system including rooms with equipment connections and associated plumbing connections. A summary of all identified cross-connections, organized by building and floor is presented in Appendix C. Tracking sheets were created for individual equipment connection locations and are presented in Appendix D.
CES – Spokane Valley, WA Mann-Grandstaff VAMC | Cross-Connection Inventory Report Doc: 2018230008 Cross-Connection Inventory Rpt.docx September 2018 | Page 2
The tracking sheets are identified by building, floor, room number (from posted numbers or design plans), and an associated ID number assigned to each cross-connection.
Example tracking sheet identification code: 1-001-C406-001
Where:
1 = Building
001 = Floor
C406 = Room Number
001 = Cross-Connection ID number
The domestic plumbing system for Building 1 was analyzed separately and an inventory of the domestic plumbing service for each floor and room is presented in Table 2 and Appendix E, respectively. Identified dead end lines are noted for each floor in Table 2.
The primary cross-connection categories identified during the inventory were the water main supplies, fire protection systems, janitor closets, equipment, irrigation connections, and hose bibs.
An inventory of all cross-connections (both hazardous and non-hazardous) is provided in
Appendix F. Appendix F includes identified cross-connections regardless if a backflow prevention assembly was installed. Hazards are noted within Appendix F for individual cross-connections.
4.0 CONCLUSIONS
A baseline inventory was created of surveyed connections to the domestic water system including rooms with equipment connections and all associated plumbing service connections. The domestic plumbing system was analyzed and inventoried separate from equipment connections and compared to existing engineering records. The majority of cross-connection issues at the Site are associated with equipment connections. Lack of backflow assemblies account for the majority of identified issues. Primary cross-connection categories are water main supplies, fire protection systems, janitor closets, equipment, irrigation connections, and hose bibs.
This Cross-Connection Inventory Report is to be used in conjunction with the accompanying CES
Corrective Action Report and Cross-Connection Control Plan to address cross-connection control deficiencies. As cross-connection deficiencies are corrected throughout the Site, this Cross-
Connection Inventory can be used as an ongoing record by MGVAMC staff to track cross-connection corrections.
