About this file
This is a Past Performance References template for offerors responding to the Dugway Proving Ground Environmental Support solicitation (W911S626RA001). Offerors must provide documentation of relevant past performance on contracts performed as a prime contractor or major subcontractor (>10% effort) within the three years prior to solicitation issuance. Up to three references are required for the prime contractor and three for each major subcontractor.
Each reference submission must include specific contract data such as contract number, award date, contract type, original and final prices with explanations of any cost growth, original and final completion dates with schedule delay explanations, and complete contact information for the Procuring Contracting Officer, Administrative Contracting Officer, and Technical Representative/COR. Offerors must provide a detailed relevancy narrative explaining how each referenced contract relates to the DPG Performance Work Statement, specifically addressing Central Hazardous Waste Storage Facility/hazardous waste management under state permits, groundwater/underground storage tank/TSCA monitoring, and performance in isolated or remote environments. Additionally, offerors must summarize technical objectives achieved and identify any negative performance documentation such as Cure Notices, Show Cause Letters, or Contract Discrepancy Reports, along with corrective actions taken. By submitting the proposal, offerors grant the Government permission to contact listed references to verify performance history and technical acceptability.
