About this file

This document contains a past performance questionnaire template and details of a federal contract solicitation for museum exhibit fabrication services. The National Park Service requires a contractor to fabricate interpretive exhibits for the New River Gorge National Park and Preserve. Offerors must submit completed performance questionnaires from past clients as part of their proposals. The questionnaire templates include a performance rating form evaluating elements such as technical performance, understanding of requirements, timeliness, cost control, and whether the reference would hire the contractor again. An additional comments form allows for further feedback. Offerors must compile and submit the completed questionnaires, though the government may also verify the information directly with references.
