About this file
This document is a Past Performance Questionnaire template issued by the 87th Contracting Squadron to collect references regarding contractor performance on previous federal contracts.
The questionnaire is designed to be completed by government representatives or customers who have directly overseen contractor performance. It requests detailed information across three main sections: offeror identification, respondent information, and contract specifics including contract title, number, amount, type, performance period, and place of performance. Respondents rate contractor performance on a five-point scale (Exceptional, Satisfactory, Marginal, Unsatisfactory, or Neutral/Unknown) across three primary evaluation categories: Performance and Supportability (five criteria covering quality of service, personnel qualifications, deliverable completion, labor turnover, and responsiveness to issues); Management (five criteria addressing overall management effectiveness, communication with government representatives, cooperation on solutions, subcontractor oversight, and quality control); and Timeliness/Schedule (five criteria evaluating recruitment speed, personnel retention, advance planning for absences, timely credentialing package submission, and complaint resolution). The questionnaire also includes a miscellaneous section asking whether the evaluator would award another contract to the firm, whether the contract was terminated for default, and whether cure notices or show cause notices were issued. Critically, any rating of Unsatisfactory, Marginal, or Exceptional requires supporting comments, with failure to provide such comments rendering the entire questionnaire void and unacceptable to the government. Completed questionnaires must be returned via email to SSgt Alexander Kouba or SSgt Noah Horn at the provided addresses.
