About this file

This is a Contractor Work Surveillance Checklist for Industrial Cleaning Services (ICS) covering CLINs 1002SV-1004SV & 1007SV-1009SV. The checklist is designed to monitor and verify contractor compliance with equipment requirements and performance standards for industrial cleaning operations.

The checklist evaluates several key areas: timely arrival of work crews, required equipment verification (including a 3,500-gallon capacity vacuum truck with 300 feet of hoses, self-contained heated power washing unit, and liquid transfer pump), proper documentation of vacuum truck weight tickets, equipment inspection by government representatives, chemical compatibility of equipment with waste materials, compliance with RCRA regulations (40 CFR 261.7), and overall work performance satisfaction. The form includes fields for recording surveillance dates, times, government representative information, and a comments section for additional observations, concluding with a signature line for the designated government representative.
