Experience/Past Performance Questionnaire
Instructions: Offerors shall furnish the following information as it pertains to this request for proposal.
Attach additional sheets as necessary.
PAST PERFORMANCE ON SIMILAR PROJECTS:
Provide information on all projects completed within the last THREE (3) years that are similar in scope to the work described in the Statement of Work. The project can be federal, state or local government, as well as private contract. Specifically, provide the following:
• Name of the project;
• Whether your company was the general contractor or a subcontractor on the project;
• The total dollar amount of the project. If your company was a subcontractor on the project then provide your subcontracted dollar amount;
• Name of the customer/client, to include a point-of-contact and phone number,
• General overview of the project, to include:
o The type of timber stand improvement, site preparation, or tree planting work o How the work was conducted; mechanical and/or herbicide treatments o The types of equipment used on the project o Provide a general overview of the number of crew members used to conduct the treatment, acres treated, and general timeframe of completion o Any other details such as those described in the EXPERIENCE ON SIMILAR
PROJECTS section of this document which could provide additional information to support the work history.
PAST PERFORMANCE NOTE: If an offeror has limited or no relevant project information, this should be addressed in the technical approach proposal. The Forest Service recognizes that an offeror may have minimal experience. In this instance, it is advised that the offeror address how they intend on insuring successful completion of the work, for example: subcontracting certain aspects of the project.
Past performance will be evaluated through reference checks provided in this section. Reference checks will be made by phone or email during the evaluation process. Reference checks will evaluate the contractors past performance with treatments similar to those in this contract.
Past performance is the most important evaluation factor due to the importance it has on the quality of contractors desired to do work under this contract.

EXPERIENCE ON SIMILAR PROJECTS:
Explain applicable experience (including personnel and equipment utilized) performing activities similar to those described in the Statement of Work.
Describe demonstrated proficiencies of treatment methods and applications such as, but not necessarily limited to:
• Using chainsaws to control native hardwood vegetation for treatments such as site preparation, midstory control, and/or crop tree release.
• Using herbicide treatments to control native hardwood vegetation for treatments such as site preparation, midstory control, and/or crop tree release.
• Keeping detailed records of daily herbicide applications.
• Planting trees in a post-timber harvest situation.
• Planting both pine and hardwood trees with long tap roots such as oaks and hickories.
• Constructing fire-line using hand tools and chainsaws.
• Conducting work in sensitive areas, or mitigating for rare, sensitive, endangered species or threatened species.
• Treating slash resulting from operations to meet other resource objectives.
Overall experience and contractor capability are very important evaluation criteria for this contract. A contractor able to demonstrate years of satisfactory or above experience and the ability to capably perform a diverse range of scheduled line items will be given high consideration.
