A. PROPOSAL SUBMISSION (Section L)

1. Instructions to Offerors

A. GENERAL INSTRUCTIONS. These instructions are a guide for preparing quotes. These instructions generally describe the type and extent of information the Offeror should provide and emphasize the significant topic areas the Offeror should address in the quote. The Offeror is expected to review the Performance Work Statement (PWS), dated 02 June 2025, found in Attachment 0001 of the Request for Proposal (RFP) for further insight into the areas that the proposal must address.

The Offeror should include sufficient information to enable the evaluators to determine the Offeror's understanding of the requirements in each of the evaluated areas. Data requested for evaluation has been clearly identified in this solicitation. Offerors should note the provision at FAR 52.212-1, "Instructions to Offerors— Commercial Products and Commercial Services." The Government intends to award a contract resulting from Request for Proposal without discussions with offerors (except clarifications described in FAR 15.306(a)). Therefore, offerors’ initial proposals should contain the best terms. The Government does, however, reserve the right to conduct discussions if determined necessary by the Contracting Officer.

Discussions will not be opened for the sole purpose of allowing an Offeror to submit data. Therefore, quotes lacking information may be considered unacceptable and may be eliminated from any further consideration. The Government reserves the right to not open discussions to allow for the submission of data that should have been submitted with the initial proposal.

The Offeror shall provide information by addressing each Factor and Subfactor in the format and sequence identified in this solicitation. The Offeror must provide information in sufficient detail to allow the Government to make an assessment of technical acceptability of the quote.

B. QUOTE FORMAT. The Offeror shall submit a proposal for this requirement as outlined in the paragraphs below. All proposal submissions shall be sent electronically to Ms. Olivia K Carroll at olivia.k.carroll2.civ@army.mil and Ms. Carly C Frye at carly.c.frye.civ@army.mil and must be received by 18 August 2025, 0800 Central Time (CT).

The Offeror is responsible for ensuring that a full and complete proposal is received by the date and time established for receipt of timely proposals and should take into consideration file size and firewalls when submitting a proposal. Any proposal, or partial proposal, received after the time and date established for receipt of timely quotes will be considered late and will not be considered for award. Absent the opening of discussions, if required, the Government will not accept any changes, additions, or deletions for evaluation purposes.

I. PAGE LIMITATIONS AND LAYOUT. Quotes must be submitted in electronic form as attachments. The Government does not assume the duty to search for data or information to cure problems discovered in quotes. The burden of providing complete and thorough information to address the elements below remains with the Offeror. Each quote shall include clear references to the corresponding paragraphs of the PWS or RFP that apply, as applicable.

II. Volume I-Technical Capability.

Subfactor 1 - Saudi Arabian Business License and VAT Certificate and CCHI and SAMA Certification.

In order to be considered for award your proposal submission shall contain legible copies of your business license and VAT certificate for the Kingdom of Saudi Arabia (KSA). Proposal shall also include proof of Council of Cooperative Health Insurance (CCHI) and Saudi Arabian Monetary Agency (SAMA) certification in the Kingdom of Saudi Arabia to provide Life and Health Insurance Services. Failure to provide this with your proposal submission shall deem your proposal non-responsive and ineligible for award.

Subfactor 2 – Technical Plan The Insurance Policy Coverage Plan/Brochure shall be specific, detailed, and complete enough to clearly and fully demonstrate a sound technical approach, and a thorough understanding of the necessary coverage for meeting performance standards and customer requirements in accordance with (IAW) the Performance Work Statement (PWS), dated 02 June 2025, Attachment 0001. III. Volume II- Past Performance Your proposal shall include three references from USG organizations that your company has provided the same or similar support to.

Reference submission shall include name, title, company, email address, phone number, and supply or services provided. For all past performance references, a Past Performance Questionnaire (Attachment 0008 in the ‘List of Attachments’ section of the Solicitation) must be completed and submitted. The Offeror shall complete Part I of the Past Performance Questionnaire and e-mail the questionnaire to:

The PCO and Contract Specialist referenced in Section 1.B. and to the Point of Contact (POC) responsible for the past/current contract. The POCs shall be instructed to electronically complete Part II of the questionnaire and e-mail the entire questionnaire to the PCO and Contract Specialist referenced in Section 1.B. no later than the proposal due date. Early submission of Past Performance data is encouraged. However, Past Performance data may be submitted up through the solicitation closing date and time. The Offeror shall submit within its Past Performance Volume a list of all the POCs who were sent a questionnaire. The POC list shall be submitted in table format to include the following fields: Solicitation Number; Company Name;

Contract Number; USG Agency; POC Last Name, First Name; POC Title; POC Telephone Number; POC email Address; and Date the questionnaire was emailed to the POC.

IV. Volume III - Price Factor The Offeror shall submit one total evaluated price utilizing Attachment 0002 - Price Matrix. This contract will not be separately priced and Offeror is responsible for completing all work as outlined in the PWS.

The Government reserves the right to require the submission of any data necessary to validate the reasonableness of an offer.

Proposed prices will be used by the Government to calculate the Total Evaluated Price (TEP). For each proposal, the Government will calculate a Total Evaluated Price by adding the total of the Base Period and the Option Periods. The Offeror shall complete the Price Matrix (Attachment 0002) as detailed below.

Offerors shall submit their proposals using the MS Excel Price Matrix (Attachment 0002). Failure to propose prices for all blue highlighted cells may result in the Offeror's proposal being considered unacceptable and ineligible for award. The Price Matrix consists of one tab, the Price Matrix Summary tab, which requires an entry for Contract Line Item Numbers (CLINs) X001 – X003 the Offeror shall input a firm-fixed price for each corresponding Task and Period of Performance as it relates to the Performance Work Statement. All firm-fixed prices are considered binding at the time of award.

Gray shaded cells represent totals or Government developed surrogates;

no action is required by Offerors in relation to gray shaded cells.

Please note that on CLIN x004 - Value Added Tax (VAT), the Government has calculated that at a rate of 15%. Make sure the VAT is not included in your proposed price for CLINs x001- x003.

For CLIN X005, the offeror is not required to input price; CLIN X005 represent Contract Data Requirements List (CDRLs) and are

UNPRICED.

All prices for this acquisition shall be stated in current U.S. dollars, rounded and displayed to two decimal places. (Current U.S. dollars shall be interpreted to include escalation where appropriate.)

In the event an offeror’s firm-fixed price is equal to zero dollars, the offeror shall input $0.00 and provide an explanation that the CLIN, corresponding Task, and Period of Performance is zero dollars for the Government in the price volume accompanying the Price Matrix Attachment 0002.

The Price Matrix is locked to prevent inadvertent changes. Offerors shall not alter the Price Matrix in any way. If an Offeror changes the Price Matrix, its proposal may be considered unacceptable and ineligible for award. The Price Matrix shall be completed as follows:

Offerors shall complete the Price Matrix Summary tab by entering its Offeror Name, Unique Entity ID (UEI), Cage Code, and proposal date in the blue highlighted cells. Offerors shall also enter amounts for CLINs x001- x003 (FFP) in the blue highlighted cells.

All cells on the Summary tab that are not highlighted in blue are either pre-populated, or they will populate automatically as they include formulas that will sum values of other cells from within the Price Matrix Summary tab.

Price proposals are assumed to represent the Offeror's best efforts, as final proposal revisions may not be requested. Any additional information considered necessary to explain the proposed pricing shall be submitted at the time of proposal submission.

Price Information: All information relating to the proposed price including all required supporting documentation must be included in the section of the proposal designated as the Price Volume. Under no circumstances shall this information and documentation be included elsewhere in the proposal.

The Offeror is required to provide all firm-fixed priced highlighted inputs for its proposed price matrix that is all-inclusive of costs to perform the work.

In addition, the offeror is required to provide all explanations of costs within its price volume when submitting its proposal. The Government will not make assumptions for any contractor proposed prices.

Certified Cost or Pricing Data/Data Other than Certified Cost or Pricing Data - The Government anticipates receiving competitive proposals and using competition to determine price reasonableness. However, the Government reserves the right to request additional other than certified cost or pricing data, necessary to determine price reasonableness if competition is not adequate for that purpose. Data other than certified cost or pricing data, if required, must be submitted in accordance with the instructions specified at FAR 15.403-5.
