About this file

Attachment F is a Responsibility Inquiry form for the Public Employees' Benefit Board (PEBB) Medical & Pharmacy Benefits RFP conducted through OregonBuys Bid No. S-44300-00014758. This form serves as a pre-award evaluation tool to assess proposer responsibility and capability to perform the contract. Proposers must demonstrate they possess the appropriate financial resources, materials, equipment, facilities, and personnel expertise necessary to fulfill all contractual obligations. The inquiry requires documentation of similar contracts completed within the last five years, including performance metrics related to schedule adherence, budget compliance, and absence of contract claims. PEBB reserves the right to request additional documentation beyond this form, including profit-and-loss statements, balance sheets, cash flow information, assets-to-liabilities ratios, bonding capacity, insurance documentation, and historical contract performance records.

The form addresses critical responsibility criteria including criminal or civil charges within the last three years involving public contracts, antitrust violations, embezzlement, or related offenses; contract terminations for default or lawsuits involving creditors or disputes; outstanding or pending judgments and financial distress indicators; bankruptcy filings or insolvency actions within the last three years; and current licensure, insurance, and authorization to conduct business in Oregon. Proposers employing 50 or more full-time workers with a prospective contract price exceeding $500,000 must provide an unexpired Pay Equity Certificate from the Department of Administrative Services. All responses must be certified as complete, accurate, and not misleading by an authorized representative of the proposer. Failure to promptly provide requested information or demonstrate responsibility may result in a PEBB finding of non-responsibility and rejection of the proposal.
