About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance on federal contracts. The form is to be completed by government respondents (such as contracting officers or program managers) who have direct knowledge of a contractor's past performance and returned to Amanda McCarthy, Contract Specialist at the Naval Air Warfare Center Training Systems Division within one week of receipt. The questionnaire applies to the Navy's P-8A Aircrew Training Device (ATD) Operational Flight Trainer (OFT) projection system upgrade solicitation (N6134026R1010-1), which is planned as a Small Business Set-Aside Commercial Competition.

The questionnaire requires identification of the contractor, program, contract details, and the respondent's information, followed by an assessment of the contractor's performance across three primary evaluation criteria: Quality of Product or Service, Schedule Performance, and Program Management and Subcontract Management. Each criterion uses a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory) with substantiating examples required for each rating. The form concludes with a summary question asking whether the respondent would award the contract to the contractor if given the choice today. Responses must be signed (electronic or handwritten signatures are acceptable) to verify completion and agreement with the information provided.
