MAINTAIN BASE PAVEMENTS CONTRACT

STATEMENT OF WORK
SCOPE OF WORK / GENERAL REQUIREMENTS
1. GENERAL SCOPE AND BACKGROUND:
a. The project is located on Candy Avenue.
b. The road currently does not provide adequate vehicle queuing along Candy Avenue, which causes traffic congestion.
c. The potable water system under the road needs to be upgraded to accommodate the work on
Candy Avenue.
d. The proposed work shall provide all labor, materials, equipment, accessories, and incidentals to demolish portions of the existing road, widen Candy Avenue, and replace the storm sewer. All work will be according to this Scope of Work (SOW), the Maintain Base Pavements Specifications, applicable codes, and as recommended by the manufacturer.
2. DESCRIPTION OF WORK REQUIRED:
a. Comply with National Pollutant Discharge Elimination System (NPDES) regulations as required in
101.30 of the General Specifications of Maintain Base Pavements. A large construction permit will be required from Wyoming Department of Environmental Quality (WDEQ) and the contractor shall be responsible for all effort and fees to obtain WDEQ approval. Prepare and adhere to the Storm Water Management Plan and Storm Water Pollution and Prevention Plan. A WDEQ construction permit, SWMP, and SWPPP are required to be submitted by the contractor to the Government for approval prior to commencement of work. The storm water management facilities shall be installed prior to any ground disturbing activities.
b. The contractor shall protect areas next to the limits of construction that are to remain such as, fencing, signage, irrigation systems, pavements, barriers, lighting, manholes, gates, and other localized site amenities and facilities. All damage or loss caused by contractor personnel or equipment shall be promptly remedied to a condition equal to or better than existing.
c. At least one lane of traffic shall be maintained during operations and access shall be immediately provided to responding emergency services and security forces vehicles. Any one lane flagging operations should be conducted to limit traffic backing up. Any revisions to the plan must be submitted for review and approval a minimum of 14 calendar days prior to advance notice of any shutdowns or phase changes.
d. Traffic shall be routed to hard, paved surfaces throughout the project. Temporary patch areas may be constructed of compacted aggregate fill and exposed to traffic up to three calendar days prior to paving. Do not utilize aggregate patch areas over weekends or days the contractor is not present without Government approval. The contractor shall be responsible for maintaining aggregate patch areas and immediately fix depressions or potholes.
e. Access must be maintained throughout the project to inbound and outbound traffic on hard, paved surfaces. An exception is that temporary aggregate access may be used to cross work zones for turning traffic until permanent pavement is placed. Temporary paved roads designed for the expected vehicle loading and duration may be utilized along Candy Avenue in lieu of flagging operations, but shall be constructed with a minimum of 6 inches of crushed aggregate base and 4 inches of asphalt pavement. A pavement design and layout plan shall be provided by the contractor for review and approval. Areas disturbed to construct temporary pavement shall be fully restored.
f. Temporary construction to be removed is not subject to testing requirements and is the responsibility of the contractor to properly install and compact. Any materials proposed to remain as part of the permanent construction will need to be constructed and tested in accordance with the specifications.
g. If any excavation or ground penetration is necessary, request and obtain 100% approval on a
Dig Permit (AF Form 103) prior to any digging, trenching, or below grade construction work. The Contractor must maintain utility marking throughout the project. Contractor is responsible for extending/updating dig permit until digging, trenching, or below grade construction work is completed. Comply with all requirements of the permit throughout construction.
h. The contractor shall immediately notify the Government of any change in conditions or modification to the project. Any changes to the work or scope must be approved by the Government and agreed to by the contractor prior to beginning the work.
i. See AF Form 66 for required submittals. The list may not be all inclusive and additional material information may be required. When doubt exists on a product, contractor shall provide a submittal.
j. The gate security building will be occupied through the duration of the project when the gate is open. Provide caution tape, signage, and barricades as necessary to block access to work areas.
k. If any welding, cutting, grinding, brazing, torch, open flame, or spark generating equipment is utilized, the contractor shall obtain a “hot work” permit from the 90 CES/CEF Fire Department prior to commencing any such work. Contractor is responsible for extending/updating “hot work” permit until such work is completed.
l. Any work or materials required to complete the work shown on the plans or included in the specifications that is not designated by a bid item shall be incidental to other work items included in the contract.
3. SECURITY REGULATIONS:
a. The Contractor shall comply with pertinent Air Force Security regulations while working on this project. Special security clearance such as Confidential, Secret, etc., is not required for the Contractor.
b. A minimum 14 day notice shall be provided to the Government to include the start date and duration of work outside the entry control facility. The Government will coordinate with security forces to provide notice of construction work and the potential of non-standard traffic movement of contractor vehicles.
4. WORK SCHEDULE:
a. Site work hours shall be from 7:30 am to 4:30 pm. Workdays are Monday through Friday, except federal holidays.
b. The contractor may be required to vacate a work area due to exercises, natural disasters, national emergencies, or security reasons. Temporary shut downs may be required for missile transfer operations. The Government will coordinate with the contractor when dates of actions are known that may impact operations.
5. PERIOD OF PERFORMANCE:
a. The period of performance is 360 calendar days. The contractor shall allow a minimum of 14 days to complete final inspection punch-list items.
b. In the case of the winter season, the contractor will notify the Government their intent to close their asphalt and concrete plants for the season due to weather. The Government will validate that the weather no longer allows work to be performed within the Specifications.
The project will be placed into suspension and the remaining number of period of performance days documented. When the suspension is lifted, a new completion date will be established based on the remaining period of performance days.
c. The contractor will prepare the work site for any periods of suspension, approved by the
Government, for any reason, to allow the entry control facility to operate at a capacity similar to that prior to the start of construction. This may include completing pavement sections out of phase sequence, placing signs or barricades to direct traffic, or opening sections of completed work to traffic. Any work outside the original scope of the project will be negotiated as a modification to the contract.
6. PLAN AND SPECIFICATION RECONCILIATION:
a. The following term equations are provided to reconcile the plans, specifications, and bid schedule for clarity:
i. AC Surface Course = Asphalt Wearing Course
ii. AC Base Course = Asphalt Leveling Course
iii. Silt Fence = Perimeter Erosion Control Fence
b. Truncated domes shall be cast iron plates cast into the ramp concrete surface.
c. Trench Backfill for all utility trenches shall conform to Section 204 unless otherwise indicated.
d. Unless otherwise specified in the contract, the Contract Documents have the following order of precedence:
i. Contract Amendments in order of issuance, most recent first
ii. Addenda
iii. Contract Document (Form DD1155)
iv. Plans
v. Statement of Work
vi. Maintain Base Pavements Specifications
