GENERAL:
This Statement of Work supplements the General Requirements within the Maintain Base Pavements Specifications in the Maintain Base Pavements Contract and more specifically defines the subject project. The contractor shall provide all engineering contract documents needed for the Government to issue a maintain Base Pavements Deliver Order as specified in the Statement of Work. The contractor shall verify all actual measurements and conduct field surveys, site investigation, and testing. The contractor shall also provide all labor, materials, and equipment necessary to complete this delivery order.
1. Description of Work:
This project consists of providing all labor, materials, equipment, transportation necessary to mill and overlay and repair soft spots on Highland Road and its intersection with Mountain Drive, as well as the approach from Hammock Road. Horizontal and vertical controls shall be maintained by the contractor at all times. The following information highlights a portion of the work, but all work is outlined in the attached spreadsheets. Work will be performed in a manner as to not inhibit operations and or access to the surrounding facilities.
a. In general, work shall include but is not limited to:
i. Comply with National Pollutant Discharge Elimination System (NPDES) regulations as required in 101.30 of the General Specifications of Maintain Base pavements dated 6 December 2018.
ii. Request and obtain 100% approval on a Dig Permit (AF Form 103) prior to any demolition/excavation work.
iii. Contractor shall follow Specifications listed in the Maintain Base Pavements contract for concrete, steel reinforcement, aggregate base course, and HMA where required.
iv. Protect area next to limits of construction that are not to be removed and replaced such as, high temperature hot water (HTHW) lines, guard rails and other localized site amenities and facilities.
v. Preparation and adherence to Storm Water management Plan and Storm Water Pollution and Prevention Plan. The plan shall be prepared by the contractor if applicable and submitted for approval prior to commencement of work.
b. Construction operations shall include, but are not limited to:
i. Soft Spot Repair
1. Removal of damaged asphalt and unsuitable sub-grade material, the removal will be the width of the lane.
2. Replacement of sub-grade and 3 inches of asphalt leveling course
ii. Header Curb Repair or Replacement
1. Removal of damaged header curb
2. Replacement of removed header curb
iii. Pavement Replacement
1. Mill entire roadway 1 inch, transition mill where new pavement will tie into existing roads i.e. Calamity Ave, Mountain Drive, and Hammock Road.
a. Some millings will be applied to the shoulder of the road
b. Excess millings will be hauled to the indicated location
2. Place 2 inches of Asphalt Wearing course taking care to ensure that edge of the final lift of asphalt is pinched down over the joint between the header curb and pavement.
3. The approach to Hammock Rd will have the header curb in the radii removed and replaced with 5 inches of base and an asphalt shoulder taper.
iv. Reflective Striping
1. 4” Reflective Roadway lane marking
2. 18” Stop Bars on the east and west ends of Highland, 1 at the approach from Hammock Road
v. Shoulder Grading
1. Strip and stockpile topsoil at the indicated location. Replace the topsoil once shoulder work is complete.
2. Grading shoulders to provide positive drainage away from road and in the downhill direction so water can flow undisturbed through the ditch. Excess shoulder material will be hauled off base.
3. Clear the culvert at Hammock Road of dirt and other debris.
4. Application of seed for disturbed areas
vi. Signs
1. Two, “Speed Limit - 30” will be replaced with new 24 in x 30 in signs.
c. All work will be accomplished in accordance with the manufacturer’s written recommendations, the latest edition of the MUTCD, F.E. Warren’s standards, and all applicable codes.
d. The Contractor will not be allowed to start work on a parking lot or road without coordination and approval from the Contracting Officer or Contracting Officer’s Representative.
e. The Contractor will provide all necessary traffic control devices to close Highland Road and the related approaches and to protect the users of the intersection Highland and Mountain Drive.
f. The Contractor must prepare the area before striping operations. This includes, but is not limited to, clearing the area of dirt and loose debris such as vegetation, sand and gravel.
g. The Contractor shall leave the site in a clean condition ready for use. All work areas will be cleaned of debris on a daily basis.
h. The Contractor shall protect all utilities, signs, bollards, header curb and amenities.
There are existing utilities that cross the road. Any utilities, sign, bollards and or amenities that are damaged during the construction operation shall be replaced or fixed by the contractor at no charge to the government.
2. Period of Performance:
The period of performance (PoP) is 90-calendar days, beginning at the issuance of the Notice to Proceed.
a. Site Work hours shall be from 7:30 a.m. to 4:30 p.m. Workdays are Monday through Friday unless site coordination is accomplished during Contracting Officer or Contracting Officer’s Representative’s working hours: Monday through Friday, 7:30 a.m. to 4:30 p.m. excluding Federal Holidays.
b. Highland Road is located near a residential area, special care should be taken to prevent any spill over or damage due to construction such as dust, excessive noise, or other materials.
3. Winter Suspension:
In the case of the winter season, the contractor will notify the Government of their intent to suspend work. 90 CES will validate that the weather no longer allows work to be performed within the Specifications. The project will be placed into suspension and document the remaining number of PoP days. When the suspension is lifted a new completion date will be established based on the remaining PoP days. The contractor may request to work during this period subject to approval by the Contracting Officer.
4. Government Furnished Property/Equipment:
None
5. Security Information:
The contractor, or their authorized representatives, shall comply with pertinent Air Force security regulations while working on this project. Special security clearances such as Confidential, Secret, etc., are not required for performance of this project.
6. Vacating the Work Area:
During exercises, natural disaster, and national emergencies or for security reasons, the contractor may be required to vacate a work area.
7. Safety:
All regulation pertaining to safety as noted in OSHA Standards, applicable AFOSH Standards, and Base Regulations shall be strictly adhered to by the contractor. Particular care shall be exercised in connection with the operation of vehicles and other equipment on the site.
8. Quality of Work:
All work shall be performed in a professional manner, suitable to the Project Engineer, and in strict compliance with the Maintain Base Pavements Specifications.
9. Schedule:
Contractor shall provide a schedule for the closure of roadways to perform the work in one application. This shall include an approved traffic management plan to ensure free flow of traffic and access to facilities. Contractor will provide the Contracting Officer a work schedule that identifies when closures will occur and time duration a minimum of 14 days prior to the closures.
