About this file

This document shows an example Official Travel Authorization form (GSA 87) and includes contractor driver requirements for FAA Academy Transportation Services.

The document outlines that contractor drivers must verify passengers have proper credentials before allowing transportation services, which can be: an FAA Welcome/New Hire Letter with photo ID, a Personal Identity Verification (PIV) Badge, or Official Travel Orders with photo ID. Drivers must electronically capture and track rider information including student name, line of business (LOB), and course number, submitting this data weekly to the FAA COR. The example form shown is marked as "EXAMPLE" and appears to be a standard GSA travel authorization form (Rev. 8/2014) that includes fields for traveler information, travel purpose, transportation methods, itinerary, and accounting details.
