About this file
This is an example document showing credential requirements and an example PIV badge image for an FAA Academy Transportation Services contract. The document outlines that contractor drivers must verify passengers have one of three acceptable forms of credentials: an FAA Welcome/New Hire Letter with photo ID, a Personal Identity Verification (PIV) Badge, or Official Travel Orders with photo ID.
The image shows a sample PIV badge marked as "EXAMPLE" in red text across the front. The badge has a blue header with "United States Government" and "FEB2018" text, a silhouette photo placeholder, DOT affiliation information, and a green chip at the bottom. The contractor is required to electronically capture and submit weekly reports to the FAA COR with passenger information including student names, line of business (LOB), and course numbers for each daily route and stop.
