| PHASE | Phase | numeric (2) | Y | Two digit number between 01 and 99. Usually 01 is set for all questions in the test bank. | NOTE: To retain the leading zero, either precede the number with an apostrophe or set the column to Text. |
| MODULE | Module | numeric (2) | Y | Two digit number between 01 and 99. Usually ties back to lesson plans but does not have to match the plan exactly. Modules should only be included if they have exam items in the plan. | |
| LESSON | Lesson | numeric (2) | Y | Two digit number between 01 and 99. Usually ties back to lesson plans but does not have to match the plan exactly. Lessons should only be included if they have exam items in the plan. | |
| LESSON_TITLE | Descriptive Title for a specific lesson. | alpha numeric (50) | N | A short description of the lesson. Use this field to document the curriculum a lesson will cover. While not required it will help users to zero in on specific questions in the question bank during exam generation as well as the objective reporting process. | |
| T.O. | Terminal Objective Number (TOBJ) | alpha numeric (3) | Y | Terminal Objective: Note that the decimal ('.') character takes up one space. It is suggested that the TOBJ and EOBJ be combined to handle objective numbers such as 1.14 or 2.33 . In the first case on would create an TOBJ of 01 and an EOBJ of 14. The second would be TOBJ = 02, EOBJ = 33. | |
| E.O. | Enabling Objective Number (EOBJ) | alpha numeric (3) | Y | Enabling Objective: Note that the decimal ('.') character takes up one space. It is suggested that the TOBJ and EOBJ be combined to handle objective numbers such as 1.14 or 2.33 . In the first case on would create an TOBJ of 01 and an EOBJ of 14. The second would be TOBJ = 02, EOBJ = 33. | |
| SECTION SUBJECT or KSATR | Descriptive Title for a Section or Descriptive Title for a Knowledge, Skill, Ability, Tool, or Resource | alpha numeric (40) | N | Use the Section Title. In cases where test items need to be created under a KSATR, use the unique KSATR verbiage from CPM. | |
| SEQ NO | The Sequence Number of a specific question within an objective (TO/EO) | number(3) | Y | The Test Question Sequence number within an objective. These should start at 1 and increment for each question in the objective. The sequence number should also restart at 1 every time the Phase, Unit, Topic, TO or EO number changes giving a "new" objective. | |
| VER NO | The Version Number of a specific question within an objective (TO/EO) | number(2) | Y | The Test Question Version number. These should be kept at 1 if possible. | |
| TYPE CD | Indicates the question type of Multiple choice, True/False, or Matching. | alpha numeric(1) | Y | Valid values are T (True/False), M (Multiple Choice), or B (Matching) | |
| SHUFFLE | Indicates if alternatives for a Multiple Choice question can be shuffled. | alpha numeric(1) | N | Valid values are Y or N (Yes/No) for Multiple Choice. Mandatory for Multiple Choice questions. Default is N (No) for True/False and Matching questions. | |
| COR ANS | The letter for the correct answer for True/False and Multiple Choice. For Matching questions, this is also the position of the correct alternative in the list of alternatives. | alpha numeric(1) | Y | If question type is True/False (T), then valid values are A (True) or B (False). If type is M (Multiple Choice) then valid values are A thru G. If Matching (B) question, this is also the 'position' of the alternative in the list of alternatives for a Matching question. Valid values for Matching questions are A - J. | NOTE: If you enter a 'G': ensure that the Multiple choice question has all 7 alternatives in the Question Alternatives columns or for Matching question, ensure that there are 7 stimuli/response combinations. |
| HEADER | Question instructions or information for matching questions. | alpha numeric(2000) | N | The instructions or information to students concerning this question. Free form text field. HEADER is mandatory and used only for Matching (B) questions. For Matching (B) questions, the HEADER should have the same text for each alternative in the set. HEADER is not used for Multiple Choice(M) and True/False(T). | NOTE: Do not include specialized characters such as the degree symbol º. These many be added to the question via Question Maintenance after the spreadsheet has been loaded. |
| QUESTION STEM ID | Matching question sequence ID | number(2) | Y/N | Valid values are 1 - 10. Sequential order from 1 to the number of stimuli/response elements for Matching (B) questions. Default is 1 for True/False (T) and Multiple choice (M). Mandatory for Matching (B) questions. | |
| QUESTION STEM (stimuli) | The question. It can be up to 2000 characters long. | alpha numeric(2000) | Y | The text of the question. Free form text field. | NOTE: Do not include specialized characters such as the degree symbol º. These many be added to the question via Question Maintenance after the spreadsheet has been loaded. |

The following is the M-16 firing cycle, assuming the rifle has been charged and the safety selection lever is on semi-automatic.
1) the trigger pushes the sear up releasing the hammer catch
2) The hammer falls forward striking the firing pin
3) The bullet is fired down the barrel
4) The expanding gas is fed back to the bolt carrier via the gas tube after the bullet passes the gas tube port and before the bullet exits the barrel at the muzzle
5) The bolt carrier is forced back in the recoil tube
a) compressing the recoil spring and
b) extracting the spent shell from the chamber
6) As the bolt is pulled back and the spent casing passes in front of the ejection port, the ejector pin pops the casing out of the port via the counter moment force generated by the pin pushing forward and the ejector claw holding on to the right side of the casing rim.
7) The decompressing recoil spring pushes the bolt carrier forward causing the bolt to strip off the next round from the magazine
8) The forward momentum causes the bolt head to lock up with the chamber locking lugs and seat the round in the chamber
| 9) the cycle starts at number 1 and will continue until the trigger is released or the magazine is emptied. | TRUE | FALSE | ||||||||||||||||
