| 1. Provide Labor Category Pricing: Use the "Summary" sheet to insert the fully burdened labor rates (FBLR) and AOH rates for the labor categories listed (column B) that are supported by the offeror's technical proposal. If the offeror's technical proposal includes labor categories outside of those listed, the offeror shall insert those labor categories (in column B) and include the FBLR for those labor categories as well. FBLR and AOH rates shall be provided for the base and all option periods. The FBLR shall be calculated with consideration to the current collective bargaining agreement, Attachment 16 - CBA 2018-2021, the Attachment 15 - Wage Determination 2015-5457 Rev 13 and include all applicable fees, overhead, profit, and taxes, including the New Mexico Gross Receipts Tax rate for Roosevelt County (Melrose). |
| 2. Provide productive hours and number of Contractor Manpower Equivalents: Use the "TEP" sheet to insert the amount of productive hours and number of CMEs proposed to meet the service requirements of CLIN X001, X002, X003, X004, X005, and X006. The number of CMEs indicated on this attachment shall be fully supported by the offeror's technical proposal. |
| 3. Provide equipment and materials pricing: Use the "TEP" sheet to insert the pricing for equipment and materials needed to perform the service requirements of CLIN X001. The equipment and materials pricing indicated on this attachment shall be fully supported by the offeror's technical proposal. Equipment and materials pricing shall be provided for the base and all option periods, as applicable. |
| 4. Provide any additional pricing, if applicable: Use the "TEP" sheet to insert any additional pricing that is applicable to the offeror's proposal. This may include additional labor categories if inserted in the "Summary" sheet or additional equipment/material pricing not already associated with CLIN X001. All additional pricing shall be fully supported by the offeror's technical proposal. Additional pricing shall be inserted as separate lines within the CLIN that it is associated with. Additional pricing shall be provided for the base and all option periods, as applicable. |
| 5. Provide Subcontract Percentages: Use the "TEP" sheet to indicate any percentages of subcontracted efforts. Subcontract percentages shall be fully supported by the offeror's technical proposal. |
| 6. The pre-determined values identified for CLIN X007, X008, X009, X010, X011, X012 and X013 are ceiling values associated with the Labor Hour/Time and Materials CLINs. Offerors shall not change the values identified in these cells. CLIN X014 is Not Separately Priced. |
| 7. Offerors are cautioned to follow the instructions listed in the instruction worksheet to complete Attachment 5, Price Proposal Workbook, properly. Offerors are responsible for ensuring that the FBLR and AOH rates inserted in the "Summary" sheet properly flow to the "TEP" sheet. The pricing indicated on the TEP sheet will be used to evaluate Total Evaluated Price. This will be done in accordance with the addendums to 52.212-1 and 52.212-2 in the solicitation and will include an evaluation of FAR 52.217-8. The offeror is requested not to provide pricing for the possible extension under FAR 52.217-8 which will be automatically calculated in the TEP tab using one half of the offeror's Option 3 pricing. |
