About this file

This document is a Past Performance Reference Questionnaire used by the General Services Administration (GSA), Public Buildings Service to evaluate contractor and individual performance on federal projects.

References must submit completed questionnaires directly to Robert Jackson, Contracting Officer (robert.n.jackson@gsa.gov) by August 25, 2026. The form is divided into two main sections: Section 1 requires the contractor to provide project details including contractor name, evaluated individual name, project title, location, client/tenant, role description, total award amount, and performance dates. Section 2 is completed by the reference and collects reference firm information, contact details, and performance ratings across three primary evaluation areas. The questionnaire assesses Similar Project experience (project size, type, delivery method, space type, and whether the project involved federal building construction, major modernization, historic register work, costs exceeding $100 million, significant facade/window restoration, CMc delivery, buildings over 300,000 square feet, or constrained urban sites); Quality performance (qualifications, documentation quality, team and resource management, schedule compliance, risk mitigation, and problem resolution); and Business Relations (client relationships and willingness to work together again). The Overall Rating section requests a comprehensive performance assessment and opportunity for additional relevant information. All questions require narrative explanations, and references must indicate whether they can be contacted for clarifications.
