About this file
This is Addendum No. 2 to Request for Quote (ARFQ) DEP25-45 for the Sullivan Refuse and Clogged Stream project issued by the West Virginia Department of Environmental Protection (WVDEP). The addendum publishes vendor questions and answers related to the project scope, which encompasses refuse pile removal, stream restoration, impoundment area excavation, and site revegetation. Key project details include the use of limestone for stream restoration items per Section 11.0 of specifications, with sandstone permitted for other items if specifications are met. Excavation volumes for refuse areas and soil cover will be calculated using pre- and post-survey measurements, and unclassified excavation quantities are calculated to finish grade. The project requires 800 tons of gravel drive rehabilitation stone for use in areas approved by WVDEP per Section 4.0 of specifications. A critical constraint is the March 31, 2025 deadline for tree clearing, with the Purchase Order and Notice to Proceed anticipated to be issued prior to that date. Saturated sediment excavated from the impoundment area is classified as refuse but may be reused as soil cover once dried and meeting specifications. Permanent seed mixture per Section 6.0 Revegetation specifications is required for disturbed areas along the stream.
Vendors must acknowledge receipt of all addenda issued with the solicitation by completing the Addendum Acknowledgement Form and submitting it with their bid, as failure to acknowledge addenda may result in bid disqualification. The addendum clarifies that only written information officially issued through addenda is binding, and verbal representations made during discussions with state personnel are not binding. All provisions of the original Agency Solicitation and other addenda not modified by this addendum remain in full force and effect.
