About this file

This is Addendum #3 to a Request for Applications issued by the New Hampshire Department of Health and Human Services, Division for Children, Youth and Families (DCYF) for Community Based Voluntary Supports services. The addendum modifies key provisions of the original solicitation RFA-2027-DCYF-01-COMMU, which seeks qualified vendors to provide case management and service coordination for moderate and high-risk families across four designated regions in New Hampshire. The program targets families recently assessed by DCYF for abuse or neglect allegations who were determined to be at moderate or high risk but did not receive a court finding of abuse or neglect. Services include ongoing case management and support for up to 120 days following completion of initial needs assessment and service plan development. The Department anticipates serving approximately 500 families per state fiscal year, with regional distribution as follows: Region 1 (Berlin and Littleton) 57 families, Region 2 (Claremont and Keene) 65 families, Region 3 (Concord, Laconia, Manchester, Southern, and Southern Telework) 285 families, and Region 4 (Conway, Rochester, and Seacoast) 93 families. The addendum also clarifies that warm handoffs with DCYF assessment workers may be conducted in person, by phone, or virtually, and establishes provisions allowing the Department to grant waivers of education and experience requirements for contractor staff upon written request and board approval.

The addendum modifies the funding amounts for State Fiscal Years 2027 and 2028 to $2,741,155 annually, totaling $5,482,310 for the two-year contract period. Flexible funding for family support services is clarified at an average of approximately $350 per family served, with allowance for variation based on individual family needs; requests exceeding $1,000 per family require Department consultation and written approval. The addendum also removes Section 1.26.1.12 from the Scope of Services and updates Appendix G regarding DCYF District Offices. All other terms, conditions, staffing requirements, service delivery standards, technical evaluation criteria, application deadlines, and contract performance metrics remain as established in the original RFA-2027-DCYF-01-COMMU solicitation documents.

New Hampshire Department of Health and Human Services Community Based Voluntary Supports

RFA-2027-DCYF-01-COMMU

Addendum #3

ADDENDUM #3

RFA-2027-DCYF-01-COMMU

Community Based Voluntary Supports (Changes are in bold, underlined and italicized text below to enable vendors to quickly recognize changes in paragraphs and/or wording.)

On March 5, 2026, the New Hampshire Department of Health and Human Services published a Request for Applications, soliciting qualified Vendors to provide support, case management, and service coordination for moderate and high-risk families to strengthen family protective factors, keep children safe, and prevent the need for subsequent involvement with the Department.

The Department is publishing this addendum to:

1. Modify Section 1.4.2. to read:

1.4.2. Objectives

The Community Based Voluntary Supports (CBVS) program prevents moderate and high-risk families assessed by DCYF from requiring subsequent involvement with the Department by connecting families with critical services and strengthening their protective capacities. After addressing any immediate needs and developing a service plan, the CBVS selected Vendor(s) will provide ongoing case management and support to enrolled families for up to 120 days. CBVS enables the Department to significantly expand the number of families who can be served and supported in a collaborative community setting.

Services are provided with an emphasis on working in partnerships with families, providing programmatic solutions to difficult family experiences, and celebrating family progress.

2. Modify Section 1.4.3. to read:

1.4.3. Covered Populations

Moderate and high-risk families who:

• Were recently assessed by DCYF for an allegation of abuse or neglect;

• As part of their CPS assessment, were determined to be at moderate or high risk of future DCYF involvement;

• Did not receive a court finding of abuse/neglect as a result of their DCYF assessment; and

• Could benefit from additional supports and be safely served in the community.

Additional referral eligibility criteria will be determined by the Department, as needed. The Department anticipates that approximately 500 families will be served per State Fiscal Year. The following anticipated number of families served by region is as follows:

• Region 1: Berlin and Littleton – 57

• Region 2: Claremont and Keene – 65

• Region 3: Concord, Laconia, Manchester, Southern, and Southern Telework –

• Region 4: Conway, Rochester, and Seacoast – 93

3. Modify Section 2.3.1. to read:

2.3.1. Funding is anticipated to be available for the resulting contract(s) as follows:

State Fiscal Year (July 1 through June 30)

Funding Amount

2027 $2,741,155

2028 $2,741,155

TOTAL $5,482,310

4. Modify Section 2.3.3. to read:

2.3.3. The Department will also allocate flexible funding for the selected Vendor(s) to support families. The flexible funding for each family served will average approximately $350. Specific requests may vary the amount of flex funds spent to support each family (e.g., some families can receive $350 and others can receive $150 or another amount needed to support the family). If the identified flexible funding need is higher than $1,000 per family collectively, the selected Vendor(s) must consult with the Department and receive written approval prior to funding the need.

5. Modify Appendix A – Scope of Services, Section 1.6. to read:

1.6. The Contractor must provide services to enrolled families for up to 120 days.

6. Modify Appendix A – Scope of Services, by adding Section 1.9.4. to read:

1.9.4. The Department may at its discretion approve a waiver of the education and experience requirements. The Contractor may request a waiver for the education and experience requirements of an above identified staff role by:

1.9.4.1. Submitting a written request to the Department that includes the following information:

1.9.4.1.1. The reason for requesting the waiver and the anticipated length of time the requested waiver will be needed;

1.9.4.1.2. Assurance that if the waiver is granted the quality of service and care to children and families will not be negatively impacted; and

1.9.4.1.3. Evidence that the Contractor’s board of directors has approved the waiver request, with a signature of the board's president or chairperson.

7. Modify Appendix A – Scope of Services, Section 1.19.2. to read:

1.19.2. The Contractor must participate in a warm handoff, that may be conducted in person, by phone, or virtually, with the Department assessment worker to introduce the family and ensure a seamless transition from the Department’s assessment to CBVS.

8. Modify Appendix A – Scope of Services, Section 1.26.1.12. by removing it in its entirety.

9. Modify Section Appendix G – DCYF District Offices in its entirety with Appendix G – DCYF District Offices – Addendum #3.
