About this file

This is Addendum #1 to a Request for Applications issued by the New Hampshire Department of Health and Human Services, Division for Children, Youth and Families (DCYF), for Community Based Voluntary Supports services. The addendum modifies the procurement timetable for RFA-2027-DCYF-01-COMMU, which seeks qualified vendors to provide intensive case management, service coordination, and support services for moderate and high-risk families across four designated regions in New Hampshire. The procurement targets approximately 700 families per state fiscal year, with required services including at least two in-person visits per week, two hours of weekly direct services utilizing Motivational Interviewing, comprehensive family assessments, collaborative service plans developed within 14 to 30 days of referral, and flexible funding for concrete household supports. Key staffing requirements include a Program Director with a bachelor's degree in social work or related field plus five years of direct experience, at least one Supervisor per region with identical qualifications, and Family Support Workers with bachelor's degrees and two years of experience maintaining maximum caseloads of ten families. The revised procurement timeline establishes a Letter of Intent submission deadline of March 12, 2026; a questions submission deadline of March 12, 2026 at noon; a Department response to questions publication date of April 2, 2026; and a vendor solicitation response due date of April 9, 2026 at noon. The contract term is effective July 1, 2026 through June 30, 2028, with potential renewal options for up to five additional years.

Total anticipated funding for the initial two-year contract period is $3,948,341 (approximately $1,974,170.50 per state fiscal year) from federal and general funds under Assistance Listing Number 93.472 (Prevention Services), with payment calculated at a daily rate of $42.68 per family served plus flexible funding for identified needs. Vendors must be registered with the New Hampshire Department of State and Department of Administrative Services, comply with all federal and state laws and civil rights requirements, maintain commercial general liability insurance of not less than $1,000,000 per occurrence and $2,000,000 aggregate, and demonstrate they are not debarred from federal or state government work. No set-aside designations for disadvantaged enterprises are specified, and no incumbent vendors or current contractors are identified. Vendors may apply to serve one or multiple regions, with preference given to applicants serving more than one region, and no match requirements apply. Technical proposals will be evaluated on a maximum of 1,000 points across ability to perform scope of services, experience and knowledge, capacity, mission alignment and quality assurance, and statewide coverage preference, with a minimum preliminary score of 575 points required for further consideration.
