About this file

This is Addendum #2 to a Request for Applications issued by the New Hampshire Department of Health and Human Services, Division for Children, Youth and Families (DCYF) for Community Based Voluntary Supports services. The RFA seeks one or more qualified vendors to provide intensive case management and service coordination for moderate and high-risk families across four designated regions in New Hampshire. Services must serve approximately 700 families per state fiscal year and include at least two in-person visits per week with evidence-based Motivational Interviewing comprising a minimum of 50 percent of direct services. Deliverables include comprehensive family assessments, collaborative service plans developed within 14 to 30 days of referral, and monthly reporting on enrollment metrics, service plan completion timelines, goal achievement rates, and recidivism data. The addendum modifies the procurement timetable, with a Letter of Intent submission deadline of March 12, 2026, a questions submission deadline of March 12, 2026 at noon, and a vendor solicitation response due date of April 14, 2026 at noon. The contract term is July 1, 2026 through June 30, 2028, with potential renewal options for up to five additional years.

Technical proposals will be evaluated on 1,000 points, with a minimum preliminary score of 575 points required for further consideration. Staffing requirements include a Program Director with a bachelor's degree in social work, counseling, psychology, or related field plus five years of direct experience; at least one Supervisor per region with identical qualifications; and Family Support Workers with bachelor's degrees and two years of experience, maintaining a maximum caseload of ten families each. Total anticipated funding for the initial two-year contract period is $3,948,341 from federal and general funds, with payment made at a daily rate of $42.68 per family served plus flexible funding averaging $500 per family for identified needs. No specific set-asides for disadvantaged enterprises are identified, and no incumbent vendors or current contractors are noted. All applicants must be registered with the New Hampshire Department of State and Department of Administrative Services, comply with federal and state laws and civil rights requirements, maintain commercial general liability insurance of not less than $1,000,000 per occurrence and $2,000,000 aggregate, and demonstrate they are not debarred from federal or state government work. Vendors may apply to serve one or multiple regions, with preference given to those applying for more than one region, and no match requirements apply.
