About this file
This is a Subcontractor/Teaming Partner Consent Form for the release of past and present performance information to a prime contractor. The form is Attachment 09 to Solicitation FA4626-26-R-0016 for the Maintain Exterior Finishes project at Malmstrom Air Force Base, Montana. The document requires major subcontractors and teaming partners identified in a proposal to provide written consent allowing the Government to disclose their past and present performance information to the prime contractor during the source selection evaluation process. This consent is necessary because subcontractor and teaming partner performance data cannot be disclosed to private parties without explicit authorization.
The form requires editing by the prime contractor with specific details including the subcontractor/teaming partner's role, the prime contractor's name, and the Government agency (Department of the Air Force). The completed and signed form must be submitted as part of Volume I - Past Performance of the proposal. The signature section requires authorization from an individual with legal authority to bind the subcontracting company, along with the company name and address. The underlying solicitation concerns construction, maintenance, and repair work on building exteriors at Malmstrom AFB, including window replacement, painting, masonry repair, and similar services, with a site visit scheduled for September 2, 2026, and questions due by September 8, 2026. The resulting contract will be a five-year Requirements type contract awarded to one offeror.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. To assist the Government's Evaluation Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Volume I – Past Performance. The following consent form requires editing by the prime contractor prior to forwarding to their respective subcontractors (see bolded text).
