About this file

This is a Past Performance Reference Sheet template designed to collect historical performance data from offerors responding to solicitation FA4626-26-R-0019 for HICS Boring and Erosion services at Malmstrom Air Force Base, Montana. Offerors must submit between three and five past performance references completed or ongoing within the five years preceding the solicitation release date. Each reference must represent projects valued at more than $40,000 and similar in scope, size, and complexity to the current requirement. References must be submitted in Volume II—Past Performance of the proposal no later than 09 September 2026 at 2:00 p.m. Mountain Daylight Time.

The template requires offerors to document contract or task order numbers, performance dates, contract dollar values, customer names and points of contact, competitive award status, contractor role (prime, subcontractor, or key personnel), and detailed descriptions of work performed. A comprehensive performance questionnaire follows, utilizing a five-point rating scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable) to evaluate past contractor performance across 24 criteria spanning personnel management, quality control, problem identification and resolution, scheduling adherence, cost proposal accuracy, small business subcontracting compliance, and overall performance suitability. Respondents must provide supporting information for each rating and answer whether the contractor received cure or show cause notices and whether the customer would award future contracts to the same contractor.

Past Performance Reference Sheet In accordance with (IAW) paragraph 4 of Section L of this solicitation, please provide between three (3) and five (5) recent and relevant past performance references. As a reminder, “recent” is defined as “completed or ongoing projects within the last five (5) years from the date of the release of this solicitation”, and “relevant” is defined as “projects similar in scope, size, and complexity to the requirements of this solicitation and valued more than $40,000.00.” Include rationale supporting your assertion of relevance.

One of the considerations in proposal evaluation is the verification of the offerors past performance on contracts or other work efforts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.

Past performance references must be provided within Volume II – Past Performance of your company’s proposal, which is to be submitted and received no later than 09 September 2026 at 2:00 p.m. Mountain Daylight Time (MDT).
