About this file

This document is a Past Performance Questionnaire issued by the USDA Forest Service for Solicitation No. 12445226R0007, seeking proposals for the GAOA Lake Winfield Scott Campground Construction project. The questionnaire is designed to evaluate a contractor's past performance history, with clients asked to complete and return the form either directly to the contractor or to the Procurement Contracting Officer at penny.zortman@usda.gov by a specified date in 2026. The form includes comprehensive rating scales for assessing contractor performance across multiple categories such as quality of product/service, cost control, schedule management, business relations, and regulatory compliance, using a rating system from Not Applicable (N/A) to Substantial Confidence (4).

The questionnaire provides detailed evaluation criteria for rating a contractor's performance, including specific metrics like quality of deliverables, adherence to schedule, management of resources, regulatory compliance, and overall performance. Clients are asked to rate the contractor on various sub-categories within each main performance area and provide an overall recommendation on whether they would select the firm for similar future requirements. The document is marked as Source Selection Information, indicating its sensitive nature and restricted use in the procurement process.
