About this file

This is a Deliverables Acceptance Document Template created by the Division of Mine Reclamation (DMR) for use in state and local projects, specifically in support of the California Department of Conservation's SMARA-4 Project being procured through the California Department of Technology's Office of Statewide Technology Procurement. The template is designed to document and formally accept project deliverables as defined during the Initiation Planning stages, with completion and acceptance reviewed during project implementation. The document requires project managers to list deliverables that match the signed and approved Project Plan, describe how each deliverable was met or satisfied, and attach relevant artifacts for the acceptance record. The template also includes sections for tracking deliverables requiring further action and documenting reasons why requirements have not been met. The final proposal submission deadline for the SMARA-4 Project is January 20, 2026 at 10:00 AM PST, with OSTP eVAQ approval noted as a non-negotiable requirement.

The template requires completion by multiple stakeholders including the Project Sponsor (Business and Technical), Contractor Project Manager, and DMR Project Manager, with all parties providing signatures and dates for approval. Key project information fields include the project name, sponsor organization, project manager contact information, impact group and scope details, audit task numbers, and service ticket references. The document serves as an official record of project deliverable acceptance and includes sections for notes, contingencies, and comments to address any issues or special circumstances related to project completion and acceptance.
