This is a template document for contractors to submit past performance narratives as part of their proposal response to the Legal Services for Unaccompanied Alien Children (UAC) solicitation (75P00126R00003). Offerors are required to submit up to three (3) separate past performance narratives describing projects of similar size, scope, and complexity that are ongoing or were completed within the past five (5) years of the solicitation date. The Government reserves the right to contact identified customer points of contact to verify the information submitted.
Each past performance narrative must address six key sections: (1) Service Provider and Contract Information, including service provider name, contract number, contract type, period of performance, total contract value, prime or subcontractor status, customer name, and customer point of contact details; (2) Project Description, covering project objectives, mission relevance, scope of work performed, and key requirements and deliverables; (3) Relevance to This Requirement, demonstrating similarity in size, scope, and complexity and identifying alignment with the solicitation's Performance Work Statement; (4) Performance Outcomes, documenting service quality, schedule performance, cost control, management effectiveness, and innovations or process improvements; (5) Results and Impact, detailing measurable outcomes, achievements, benefits to the customer, and any awards or recognition received; and (6) Challenges and Corrective Actions, describing any performance issues encountered, resolution actions taken, and lessons learned. This template is referenced in RFP Section L.6.2 for additional requirements.

Attachment I – Contractor Past Performance Narrative Template Offerors shall use this Contractor Past Performance Narrative Template to submit up to three (3) past performance narratives describing projects of similar size, scope, and complexity to this requirement that are ongoing or were completed within the past five (5) years of the date of the solicitation. A separate form must be submitted for each project. Reference RFP Section L.6.2 for additional requirements. The Government reserves the right to contact the identified customer point of contact to verify past performance information submitted by the offeror.

2. Project Description: Provide a brief description of the project, including objectives and mission relevance; scope of work performed; and key requirements and deliverables.
3. Relevance to This Requirement: Describe how the effort is similar in size, scope, and complexity, and identify key tasks or requirements that align with the solicitation PWS.
4. Performance Outcomes: Describe the quality of the services delivered; the contractor’s ability to meet schedule requirements (e.g., timeliness, milestones met); the contractor’s ability to control costs (if applicable); management effectiveness (e.g., staffing, communication, issue resolution); and innovations or process improvements in service delivery.
5. Results and Impact: Describe measurable outcomes, achievements, or benefits to the customer and any awards, recognition, or exceptional ratings received.
6. Challenges and Corrective Actions: Describe any performance issues encountered, actions taken to resolve performance issues, and lessons learned (if applicable)
