GENERAL REQUIREMENTS
PART 1 - GENERAL
1.1 SAFETY REQUIREMENTS
A. Refer to section 01 35 26, SAFETY REQUIREMENTS for safety and infection control requirements.
1.2 GENERAL INTENTION
A. Contractor shall completely prepare site for building operations, including demolition coordinate with Government’s equipment provider, and furnish labor and materials and perform work for Replace Linear Accelerator as required by drawings and specifications.

The contractor shall provide all necessary work including the renovation of linear accelerator room, the conversion of a room into a control room, the addition of a mechanical room, the addition of a data server room, replace ceiling and flooring, rewire, provide new lead as required, install new air compressor, air dryer system, and piping, install (2) 2-ton HVAC units, (1) new dedicated fan coil unit for gantry cooling, new duct work, install new drywall, finishes and casework and paint, revise fire protection as required, install ceiling mounted lift system with a minimum carrying capacity of 770 lbs., provide structural bracing, foundation, and supports, including those based on VA, City of Los Angeles, and State of California seismic requirements.
Provide electrical power requirements for lift motor, PDU, linear accelerator and associated equipment. Provide new controls and signaling systems for communications and security scope associated with this project. Contractor shall coordinate with equipment providers and existing equipment shall be removed by the contractor. Contractor shall also coordinate with equipment provider and install new equipment per manufacturer's requirements.
B. Visits to the site by Bidders may be made only by appointment with the Medical Center Engineering Officer.
C. Offices of Triple C – The Architects and Engineers, as Architect-Engineers, will render certain technical services during construction. Such services shall be considered as advisory to the Government and shall not be construed as expressing or implying a contractual act of the Government without affirmations by Contracting Officer or his duly authorized representative.
D. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access.
E. All construction is to be performed in one phase during normal working hours as much as feasible. Off-hours premium time is included for noise abatement and utility shutdowns only.
Any item(s) to be salvaged by the owner are to be removed by the owner from the site before construction begins. contractor to provide dumpsters for disposal of construction debris and materials. The construction contractor will be required to submit construction waste debris reports to include weight of construction waste, weight of any recycled materials and the cost for removing construction waste. The construction contractor shall comply with VA and Medical Center guidance to ensure infection controls during the construction.
1. Normal Working Hours are 0630 to 1530, Monday to Friday.
2. For working outside these hours requires prior written approval from the COR. Due to the work site location, it is anticipated that off-hours work will be required to perform this project work to manage noise and vibration impacts to Veterans Patients and Medical Staff performing care.
F. All testing shall be by certified testing laboratories subcontracted by the Contract. The Government shall not be required to contract testing for any trade.
G. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access.
H. The Government does not provide courtesy, pre-inspections of area during the construction.
The Contractor shall request progress inspections and communicate requests for inspections at least one week in advance.
I. Contractor shall appoint a third-party individual, company, or firm to act as the Commissioning Agent. The Commissioning Agent is the designated person, company, or entity that plans, schedules, and coordinates the commissioning team to implement the commissioning process.
See 01 91 00 for more details.
J. Comprehensive 3rd party inspections, testing and written reports for all Divisions (paid by the General Contractor). Contractor shall employ third party inspection consultants to perform all inspections required by this contract. The VA has no inspection capability. Any reference to VA inspections shall be by a third-party inspector qualified to perform the work. Credentials shall be

01 00 00 - 3 supplied to the VA/AE as required by this contract. All inspections, testing, certifications, and permits are to be paid for by the General Contractor. The VA will not pay for or perform any required inspections. VA inspections are for VA use only, when performed, and do not reduce the inspection burden on the Contractor. Third party inspection services must be submitted for acceptance by the VA after award. Submission to include resumes of inspectors, with alternates, projected days on site, qualifications of inspectors showing similar projects, related portfolio, and description of communication protocol with Owner.
K. Contractor shall notify VA COR minimum (7) days in advance of each third-party inspection to allow coordination with VA personnel.

2. Minimum inspections required:
a. Structural, Mechanical, Electrical, Plumbing, Fire Protection, low voltage systems, communications and security systems, controls system integration, commissioning support, Framing, interior finishes, and doors/hardware
L. Contractor to provide Submittal Exchange®, Autodesk A360® Project Site dedicated for this project, or approved equal (substitutions must be submitted with Contractor Bid), for the duration of the contract. Contractor will provide VA access to the site. Contractor will keep this site current and will use it for all submittals, RFIs, change documents, Record Drawing, electronic file management, and communication logs for the duration of the contract.
M. Contractor to provide Infection Control Plan as required by Memorandum 111-40, Subj: VHA Directive 7715 Safety and Health During Construction dated April 6, 2017. Submit to COR for approval prior to the start of work.
N. Prior to commencing work, general contractor shall provide proof that an OSHA designated “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. CP will have authority letter.
O. General Contractor’s Superintendent assigned to this project shall not be assigned to any other project and shall only have duties on one project at a time. Site Safety and Health Officer (SSHO) has to be a separate qualified individual from the Prime Contractor’s Superintendent or Quality Control Manager with duties only as the SSHO. See 01 35 26 SAFETY REQUIREMENTS for more information on roles and responsibilities for General Contractor’s staff.
P. Per VA Directive 1085, smoking, vaping, and smokeless tobacco are prohibited on the grounds of VA facilities, including in vehicles. This Directive applies to all Prime Contractor and Subcontractor employees.
Q. Pricing Proposal transparency requirements: this project requires pricing proposals to include breakdown pricing for the General Contractor and all subcontractors. All price proposals, including subcontractor pricing must include a breakdown of pricing per the format included with tis specification. Third-party inspections must be included as a line item in the price proposal.
See example at the end of GENERAL REQUIREMENTS section.
R. Refer to specifications for installation requirements for each Division. Refer to drawings for estimated quantities and schematic layouts/locations. Contractor to provide a complete, commissioned, functioning product. The VA is not required to inspect or install any portion of this project. Contractor bears all installation and testing costs. Contractor to provide all testing reports prior at the start of commissioning. This project will require night and weekend work.

Renovation includes requirements for Testing and Balancing the HVAC for the existing and new systems configuration.
S. Please see Appendix B for the Asbestos Survey performed in the Area of Work. ACM is not shown to be present in the area of work. Should unforeseen ACM be detected Traditional and Glovebag Abatement methods, as stipulated in Specification Sections 02 82 11, 02 82 13.13, and 02 82 13.19, are the only allowable methods of abatement. No alternate abatement methods will be considered.
1.3 STATEMENT OF BID ITEM(S)
A. ITEM I, GENERAL CONSTRUCTION: Work includes general construction, alterations, renovations, infrastructure, necessary removal of existing structures and construction and certain other items. Work includes renovation of Linear Accelerator Equipment and the conversion of a room into a control room, the addition of a mechanical room, and the addition of a data server room. The expected construction period duration for each building includes all pre-construction activity and post-construction testing, third-party inspections, and commissioning. Contractor shall submit a comprehensive schedule of work in compliance with 01 32 16.15 PROJECT SCHEDULES (SMALL PROJECTS DESIGN/BID/BUILD). Contractor to perform Commissioning of all new systems and existing systems impacted by the project according to requirements in sections: 01 91 00 – GENERAL COMMISSIONING
REQUIREMENTS; 23 08 00 – COMMISSIONING OF HVAC SYSTEMS; 26 08 00 –
COMMISSIONING OF ELECTRICAL SYSTEMS; and 27 08 00 – COMMISSIONING OF COMMUNICATIONS SYSTEMS. A detailed commissioning plan is required to ensure that all devices are calibrated, programmed, and all graphic displays are accurate and approved by applicable VA staff.
B. ITEM II, ELECTRICAL AND COMMUNICATIONS: Work includes all labor, material, equipment, and supervision to perform the required electrical construction work on this project including removing existing and providing new electric service for the new Radixact system, all associated equipment, and all new equipment. Scope includes providing wiring, conduits & devices for power distribution and lighting, and conduit and all communications cabling and devices for the work area. Electrical work furnished and installed by construction contractor shall include, but is not limited, to emergency stop buttons, door switch and reset, system status indicators, thermostats, temperature sensors, junction boxes, receptacles, lasers, power and signal conduits, electrical trenches, lighting, fire safety equipment, physics conduit, power for CCTV and intercom system. The Contractor will provide communications, IT and security systems, detection and alarm systems, access and surveillance systems, and automation and control systems and wiring.
ITEM III, MECHANICAL AND PLUMBING: Work includes all labor, material, equipment, and supervision to perform the required Mechanical and Plumbing construction work on this project including (but not limited to): modify the existing fire sprinkler system as required; provide new plumbing fixtures, piping and drains; provide new air compressor, dryer, receiver, piping, valves and associated devices; provide (2) dedicated 2 ton HVAC units for the Radixact server and mechanical rooms; provide new dedicated HVAC system serving the new Linear Accelerator gantry and ductwork in the vault and other renovated areas. New and existing HVAC units shall have 24/7 monitors and shall integrate with the existing Facility Management System.
Contractor is required to Test and Balance (TAB) and commission new and existing HVAC systems per 23 05 93 and 01 91 00. This work requires preconstruction TAB readings to establish baseline performance for all existing HVAC systems serving the space and TAB readings for all new CRAC units and new Air Handling Unit. Existing systems shall be rebalanced to preconstruction readings. For fire suppression, contractor shall reconfigure and provide additional sprinkler heads to the existing building automatic sprinkler system as required. Contractor shall coordinate sprinkler coverage requirements with all temporary ICRA barriers to ensure all areas are properly protected during construction.
C. DEDUCT ALTERNATE NO.1: Eliminate patient lift scope of work.

The work shall be performed a maximum of 180 days.
The expected construction period duration for each building includes all pre-construction activity and post-construction testing, third-party inspections, and commissioning. Contractor shall submit a comprehensive schedule of work in compliance with 01 32 16.15 PROJECT SCHEDULES (SMALL PROJECTS DESIGN/BID/BUILD). Contractor to perform Commissioning of all new systems and existing systems impacted by the project according to requirements in sections: 01
91 00 – GENERAL COMMISSIONING REQUIREMENTS; 23 08 00 – COMMISSIONING OF
HVAC SYSTEMS; 26 08 00 – COMMISSIONING OF ELECTRICAL SYSTEMS; and 27 08 00 – COMMISSIONING OF COMMUNICATIONS SYSTEMS. A detailed commissioning plan is required to ensure that all devices are calibrated, programmed, and all graphic displays are accurate and approved by applicable VA staff.
1.4 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR
A. Drawings and contract documents may be obtained from the website where the solicitation is posted. Additional copies will be at Contractor's expense,
1.5 CONSTRUCTION SECURITY REQUIREMENTS
A. Security Plan:
1. The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.
2. The General Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations.
B. Security Procedures:
1. General Contractor and all Subcontractor personnel shall follow current VA badging procedures.
2. General Contractor’s employees shall not enter the project site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site.
3. Before starting work the General Contractor shall give one week’s notice to the Contracting Officer so that security arrangements can be provided for the employees.
This notice is separate from any notices required for utility shutdown described later in this section.
4. No photography of VA premises is allowed without written permission of the Contracting Officer.
5. VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of the Contracting Officer.
C. Guards:
1. The General Contractor shall provide unarmed guards at the project site.
2. The Contractor shall provide the guards and VA police with communication devices as directed.
3. The general Contractor shall install equipment for recording guard rounds to ensure systematic checking of the premises.
D. Key Control:
1. Contractor will be issued VA Keys and will require a signature confirming that they received the key(s) listed on signature form. General Contractor is responsible for returning these keys upon separation of project or at any time the VA requests. Lost or misplaced keys are to be reported to VA COR immediately and additional costs to replace keys and all cores associated with that key series will be calculated at that time with the VA Locksmith Shop and charged to the contractor.
2. The General Contractor may not duplicate Government issued keys.

3. The General Contractor shall provide duplicate keys and lock combinations to the Contracting officers representative (COR) for the purpose of security inspections of every area of project including toolboxes and parked machines and take any emergency action.
4. The General Contractor shall turn over all permanent lock cylinders to the VA locksmith for permanent installation. See Section 08 71 00, DOOR HARDWARE and coordinate.
E. Document Control:
1. Before starting any work, the General Contractor/Sub Contractors shall submit an electronic security memorandum describing the approach to following goals and maintaining confidentiality of “sensitive information”.
2. The General Contractor is responsible for safekeeping of all drawings, project manual and other project information. This information shall be shared only with those with a specific need to accomplish the project.
3. Certain documents, sketches, videos or photographs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”. Secure such information in separate containers and limit the access to only those who will need it for the project. Return the information to the Contracting Officer upon request.
4. These security documents shall not be removed or transmitted from the project site without the written approval of Contracting Officer.
5. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.
6. Notify Contracting Officer and Site Security Officer immediately when there is a loss or compromise of “sensitive information”.
7. All electronic information shall be stored in specified location following VA standards and procedures using an Engineering Document Management Software (EDMS).
a. Security, access, and maintenance of all project drawings, both scanned and electronic shall be performed and tracked through the EDMS system.
b. “Sensitive information” including drawings and other documents may be attached to e-mail provided all VA encryption procedures are followed.
F. Motor Vehicle Restrictions
1. Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitted 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.
2. A limited number of (2 to 5) permits shall be issued for General Contractor and its employees for parking in designated areas only.
1.6 OPERATIONS AND STORAGE AREAS
A. The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting Officer. The Contractor shall hold and save the Government, its officers, and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.
B. Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Contractor only with the approval of the Contracting Officer and shall be built with labor and materials furnished by the Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Contractor and shall be removed by the Contractor at its expense upon completion of the work. With the written consent of the Contracting Officer, the buildings and utilities may be abandoned and need not be removed.
C. The Contractor shall, under regulations prescribed by the Contracting Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting Officer. When materials are transported in prosecuting the work, vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer of the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Contractor shall protect them from damage. The Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.

D. Working space and space available for storing materials shall be as determined by the COR.
1. Exterior Working space are to be fully maintained to include securing fencing, providing fabric screening on fencing, controlling dust through application of water as required, protecting the site with the use of rock, gravel material to control dust and erosion.
E. Workers are subject to rules of Medical Center applicable to their conduct.
F. Execute work so as to interfere as little as possible with normal functioning of Medical Center as a whole, including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel, except as permitted by COR where required by limited working space.
1. Do not store materials and equipment in other than assigned areas.
2. Schedule delivery of materials and equipment to immediate construction working areas within buildings in use by Department of Veterans Affairs in quantities sufficient for not more than two workdays. Provide unobstructed access to Medical Center areas required to remain in operation.
3. Where access by Medical Center personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and safety requirements.
G. Utilities Services: Where necessary to cut existing pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems (except telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by COR . All such actions shall be coordinated with the COR or Utility Company involved:
1. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, for such items as water, sewer, electricity, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.
H. Building(s) No.(s) 500 will be occupied during performance of work
1. Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered. Contractor shall permit access to Department of Veterans Affairs personnel and patients through other construction areas which serve as routes of access to such affected areas and equipment. These routes whether access or egress shall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc. to facilitate patient and staff access. Coordinate alteration work in areas occupied by Department of Veterans Affairs so that Medical Center operations will continue during the construction period.
2. Immediate areas of alterations not mentioned in preceding Subparagraph 1 will be temporarily vacated while alterations are performed.
I. Construction Fence: Before construction operations begin, Contractor shall provide a chain link construction fence, 2.1m (seven feet) minimum height, around the construction area indicated on the drawings. Provide gates as required for access with necessary hardware, including hasps and padlocks. Fasten fence fabric to terminal posts with tension bands and to line posts and top and bottom rails with tie wires spaced at maximum 375mm (15 inches). Bottom of fences shall extend to 25mm (one inch) above grade. Remove the fence when directed by
COR.
J. When a building and/or construction site is turned over to Contractor, Contractor shall accept entire responsibility including upkeep and maintenance therefore:

1. Contractor shall maintain a minimum temperature of 4 degrees C (40 degrees F) at all times, except as otherwise specified.
2. Contractor shall maintain in operating condition existing fire protection and alarm equipment. In connection with fire alarm equipment, Contractor shall make arrangements for pre-inspection of site with Fire Department or Company (Department of Veterans Affairs or municipal) whichever will be required to respond to an alarm from Contractor's employee or watchman.
K. Utilities Services: Maintain existing utility services for Medical Center at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uninterrupted services. Where necessary to cut existing water, steam, gases, sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (including telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by COR.
1. No utility service such as water, gas, steam, sewers or electricity, or fire protection systems and communications systems may be interrupted without prior approval of COR.
Electrical work shall be accomplished with all affected circuits or equipment de-energized. When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without a detailed work plan, the Medical Center Director’s prior knowledge and written approval. Refer to specification Sections 26
05 11, REQUIREMENTS FOR ELECTRICAL INSTALLATIONS, 27 05 11
REQUIREMENTS FOR COMMUNICATIONS INSTALLATIONS and 28 05 00, COMMON WORK RESULTS FOR ELECTRONIC SAFETY AND SECURITY for additional requirements.
2. Contractor shall submit a request to interrupt any such services to COR, in writing, 7 days in advance of proposed interruption. Request shall state reason, date, exact time of, and approximate duration of such interruption.
3. Contractor will be advised (in writing) of approval of request, or of which other date and/or time such interruption will cause least inconvenience to operations of Medical Center.
Interruption time approved by Medical Center may occur at other than Contractor's normal working hours.
4. Major interruptions of any system must be requested, in writing, at least 15 calendar days prior to the desired time and shall be performed as directed by the COR.
5. In case of a contract construction emergency, service will be interrupted on approval of COR. Such approval will be confirmed in writing as soon as practical.
6. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, for such items as water, sewer, electricity, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.
L. Abandoned Lines: All service lines such as wires, cables, conduits, ducts, pipes and the like, and their hangers or supports, which are to be abandoned but are not required to be entirely removed, shall be sealed, capped or plugged at the main, branch or panel they originate from.
The lines shall not be capped in finished areas, but shall be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or partitions; so that they are completely behind the finished surfaces.
M. To minimize interference of construction activities with flow of Medical Center traffic, comply with the following:
1. Keep roads, walks and entrances to grounds, to parking and to occupied areas of buildings clear of construction materials, debris and standing construction equipment and vehicles.
N. Coordinate the work for this contract with other construction operations as directed by COR.
This includes the scheduling of traffic and the use of roadways, as specified in Article, USE OF
ROADWAYS.

1.7 ALTERATIONS
A. Survey: Before any work is started, the Contractor shall make a thorough survey with the COR and a representative of VA Supply Service, of areas of buildings in which alterations occur and areas which are anticipated routes of access, and furnish a report, signed by both, all three, to the Contracting Officer. This report shall list by rooms and spaces:
1. Existing condition and types of resilient flooring, doors, windows, walls, and other surfaces not required to be altered throughout affected areas of building.
2. Existence and conditions of items such as plumbing fixtures and accessories, electrical fixtures, equipment, venetian blinds, shades, etc., required by drawings to be either reused or relocated, or both.
3. Shall note any discrepancies between drawings and existing conditions at site.
4. Shall designate areas for working space, materials storage, and routes of access to areas within buildings where alterations occur, and which have been agreed upon by Contractor and COR.
B. Any items required by drawings to be either reused or relocated or both, found during this survey to be nonexistent, or in opinion of COR and/or Supply Representative , to be in such condition that their use is impossible or impractical, shall be furnished and/or replaced by Contractor with new items in accordance with specifications which will be furnished by Government. Provided the contract work is changed by reason of this subparagraph B, the contract will be modified accordingly, under provisions of clause entitled "DIFFERING SITE CONDITIONS" (FAR 52.236-2) and "CHANGES" (FAR 52.243-4).
C. Re-Survey: Thirty days before expected partial or final inspection date, the Contractor and COR together shall make a thorough re-survey of the areas of buildings involved. They shall furnish a report on conditions then existing, of resilient flooring, doors, windows, walls, and other surfaces as compared with conditions of same as noted in first condition survey report:
1. Re-survey report shall also list any damage caused by Contractor to such flooring and other surfaces, despite protection measures; and, will form basis for determining extent of repair work required of Contractor to restore damage caused by Contractor's workers in executing work of this contract.
D. Protection: Provide the following protective measures:
1. Wherever existing roof surfaces are disturbed they shall be protected against water infiltration. In case of leaks, they shall be repaired immediately upon discovery.
2. Temporary protection against damage for portions of existing structures and grounds where work is to be done, materials handled, and equipment moved and/or relocated.
3. Protection of interior of existing structures at all times, from damage, dust, and weather inclemency. Wherever work is performed, floor surfaces that are to remain in place shall be adequately protected prior to starting work, and this protection shall be maintained intact until all work in the area is completed.
1.8 DISPOSAL AND RETENTION
A. Materials and equipment accruing from work removed and from demolition of buildings or structures, or parts thereof, shall be disposed of as follows:
1. Reserved items which are to remain property of the Government are identified by attached tags or noted on drawings or in specifications as items to be stored. Items that remain property of the Government shall be removed or dislodged from present locations in such a manner as to prevent damage which would be detrimental to re-installation and reuse. Store such items where directed by COR.
2. Items not reserved shall become property of the Contractor and be removed by Contractor from Medical Center.
3. Items of portable equipment and furnishings located in rooms and spaces in which work is to be done under this contract shall remain the property of the Government. When rooms and spaces are vacated by the Department of Veterans Affairs during the alteration period, such items which are NOT required by drawings and specifications to

01 00 00 - 10 be either relocated or reused will be removed by the Government in advance of work to avoid interfering with Contractor's operation.
4. PCB Transformers and Capacitors: The Contractor shall be responsible for disposal of the Polychlorinated Biphenyl (PCB) transformers and capacitors. The transformers and capacitors shall be taken out of service and handled in accordance with the procedures of the Environmental Protection Agency (EPA) and the Department of Transportation (DOT) as outlined in Code of Federal Regulation (CFR), Titled 40 and 49 respectively.
The EPA's Toxic Substance Control Act (TSCA) Compliance Program Policy Nos. 6- PCB-6 and 6-PCB-7 also apply. Upon removal of PCB transformers and capacitors for disposal, the "originator" copy of the Uniform Hazardous Waste Manifest (EPA Form 8700-22), along with the Uniform Hazardous Waste Manifest Continuation Sheet (EPA Form 8700-22A) shall be returned to the Contracting Officer who will annotate the contract file and transmit the Manifest to the Medical Center's Chief.

1.9 RESTORATION
A. Remove, cut, alter, replace, patch and repair existing work as necessary to install new work.
Except as otherwise shown or specified, do not cut, alter or remove any structural work, and do not disturb any ducts, plumbing, steam, gas, or electric work without approval of the COR.
Existing work to be altered or extended and that is found to be defective in any way, shall be reported to the COR before it is disturbed. Materials and workmanship used in restoring work, shall conform in type and quality to that of original existing construction, except as otherwise shown or specified.
B. Upon completion of contract, deliver work complete and undamaged. Existing work (walls, ceilings, partitions, floors, mechanical and electrical work, lawns, paving, roads, walks, etc.)
disturbed or removed as a result of performing required new work, shall be patched, repaired, reinstalled, or replaced with new work, and refinished and left in as good condition as existed before commencing work.
C. At Contractor's own expense, Contractor shall immediately restore to service and repair any damage caused by Contractor's workers to existing piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications systems (including telephone) which are not scheduled for discontinuance or abandonment.
D. Expense of repairs to such utilities and systems not shown on drawings or locations of which are unknown will be covered by adjustment to contract time and price in accordance with clause entitled "CHANGES" (FAR 52.243-4) and "DIFFERING SITE CONDITIONS" (FAR 52.236-2).

1.10 PROFESSIONAL SURVEYING SERVICES
A. A registered professional land surveyor or registered civil engineer whose services are retained and paid for by the Contractor shall perform services specified herein and in other specification sections. The Contractor shall certify that the land surveyor or civil engineer is not one who is a regular employee of the Contractor, and that the land surveyor or civil engineer has no financial interest in this contract.
1.11 AS-BUILT DRAWINGS
A. The contractor shall maintain two full size sets of as-built drawings which will be kept current during construction of the project, to include all contract changes, modifications, and clarifications.
B. All variations shall be shown in the same general detail as used in the contract drawings. To ensure compliance, as-built drawings shall be made available for the COR review, as often as requested.
C. Contractor shall deliver two approved completed sets of as-built drawings in the electronic version (scanned PDF) to the COR within 15 calendar days after each completed phase and after the acceptance of the project by the COR.
D. Paragraphs A, B, & C shall also apply to all shop drawings.
E. Contractor is to provide a LIDAR survey of the renovated area and the interstitial spaces above and below the affected area as part of the close out process. This LIDAR survey will serve as documentation that the work was completed and shall work in conjunction with the as-built drawings. The unregistered (raw) point cloud data and registered, linked, point cloud model shall be turned over the Government at project close out.
1.12 WARRANTY MANAGEMENT
A. Warranty Management Plan: Develop a warranty management plan which contains information relevant to FAR 52.246-21 Warranty of Construction in at least 30 days before the planned pre-warranty conference, submit four sets of the warranty management plan. Include within the warranty management plan all required actions and documents to assure that the Government receives all warranties to which it is entitled. The plan must be in narrative form and contain sufficient detail to render it suitable for use by future maintenance and repair personnel, whether tradesman, or of engineering background, not necessarily familiar with this contract. The term “status” as indicated below must include due date and whether item has been submitted or was approved. Warranty information made available during the construction phase must be submitted to the Contracting Officer for approval prior to each monthly invoice for payment.
Assemble approved information in a binder and turn over to the Government upon acceptance of the work. The construction warranty period will begin on the date of the project acceptance and continue for the product warranty period. A joint 4 month and 9-month warranty inspection will be conducted, measured from time of acceptance, by the Contactor and the Contracting Officer. Include in the warranty management plan, but not limited to, the following:
1. Roles and responsibilities of all personnel associated with the warranty process, including points of contact and telephone numbers within the company of the Contractor, subcontractors, manufacturers, or suppliers involved.
2. Furnish with each warranty the name, address, and telephone number of each of the guarantor’s representatives nearest project location.
3. Listing and status of delivery of all Certificates of Warranty for extended warranty items, to include roofs, HVAC balancing, pumps, motors, transformers and for all commissioned systems such as fire protection and alarm systems, sprinkler systems and lightning protection systems, etc.
4. A list for each warranted equipment item, feature of construction or system indicating:
a. Name of item.
b. Model and serial numbers.
c. Location where installed.

d. Name and phone numbers of manufacturers and suppliers.
e. Name and phone numbers of manufacturers or suppliers.
f. Names, addresses and phone numbers of sources of spare parts.
g. Warranties and terms of warranty. Include one-year overall warranty of construction, including the starting date of warranty of construction. Items which have extended warranties must be indicated with separate warranty expiration dates.
h. Starting point and duration of warranty period.
i. Summary of maintenance procedures required to continue the warranty in force.
j. Cross-reference to specific pertinent Operation and Maintenance manuals.
k. Organizations, names, and phone numbers of persons to call for warranty service.
l. Typical response time and repair time expected for various warranted equipment.
5. The plans for attendance at the 4 and 9-month post construction warranty inspections conducted by the government.
6. Procedure and status of tagging of all equipment covered by extended warranties.
7. Copies of instructions to be posted near selected pieces of equipment where operation is critical for warranty and/or safety reasons.
B. Performance Bond: The Performance Bond must remain effective throughout the construction period
1. In the event the Contractor fails to commence and diligently pursue any construction warranty work required, the Contracting Officer will have the work performed by others, and after completion of the work, will charge the remaining construction warranty funds of expenses incurred by the Government while performing the work, including, but not limited to administrative expenses.
2. In the event sufficient funds are not available to cover the construction warranty work performed by the Government at the contractor’s expenses, the Contracting Officer will have the right to recoup expenses from the bonding company.
3. Following oral or written notification of required construction warranty repair work, the Contractor shall respond in a timely manner. Written verification will follow oral instructions. Failure to respond will be cause for the Contracting Officer to proceed against the Contractor.
C. Pre-Warranty Conference: Prior to contract completion, and at a time designated by the Contracting Officer, the Contractor shall meet with the Contracting Officer to develop a mutual understanding with respect to the requirements of this section. Communication procedures for Contractor notification of construction warranty defects, priorities with respect to the type of defect, reasonable time required for Contractor response, and other details deemed necessary by the Contracting Officer for the execution of the construction warranty will be established/ reviewed at this meeting.
