About this file

This is a cost sheet attachment for a Request for Bid issued by Salt Lake City Corporation for a single purchase of a Runway Closure Marker as detailed in the associated Scope of Work document (SLCI26088 Attachment 2). The bid solicitation is for a one-time purchase contract, and bidders must complete the cost sheet by providing pricing on the designated line item. The contract will be awarded to the offeror submitting the lowest total cost. All entries in highlighted cells are mandatory, and any deviation from the prescribed cost sheet format may result in the bidder's response being deemed non-responsive. Bidders must provide their company name, business structure classification, authorized representative information, and principal office contact details.

Pricing must include all shipping, handling, and associated costs within the single line-item price quoted for the Runway Closure Marker. The cost sheet incorporates mandatory City Ethics Requirements that bidders must acknowledge and agree to comply with, including representations regarding prohibited conduct such as illegal gifts, unauthorized commission arrangements, and breaches of the City's conflict of interest ordinance (Chapter 2.44, Salt Lake City Code). Bidders must verify their understanding and agreement with the City's ethics standards, including the prohibition against offering gifts or incentives to City employees, as non-compliance or failure to acknowledge these requirements will result in bid disqualification.
