About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico, for use in the Abiquiu Hydroelectric Power Plant Crane Replacement project. The form requires bidders and contractors to certify their eligibility to participate in the procurement by affirming that neither they nor their principals are currently debarred, suspended, proposed for debarment, or voluntarily excluded from covered transactions by any federal, state, or local agency. Bidders must also confirm that within the three-year period preceding certification, they have not been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, or other offenses indicating lack of business integrity. Additionally, bidders must certify they are not indicted or criminally charged with such offenses, are not current or former County employees, are not immediate family members of County employees or public officials, and have not had public contracts terminated for cause or default within the preceding three-year period.

The form permits bidders unable to certify any statements to provide written explanations for review, though such explanations do not necessarily preclude consideration for award. Bidders must also confirm that all proposed subcontractors are not currently suspended or debarred from conducting business with any city, state, county, or federal government entities. The certification must be signed by an authorized representative and submitted with the bid response. Falsification of any statement constitutes grounds for rejection of the proposal or rescission of any contract award.
