About this file

This document is a Certificate of Interested Parties and Conflict of Interest Questionnaire for the City of Lubbock, Texas, detailing legal requirements for business entities entering into contracts with local government. The document outlines two key compliance obligations: the Certificate of Interested Parties (Form 1295) and the Conflict of Interest Questionnaire (CIQ), which are mandatory for contracts meeting specific criteria, such as requiring City Council action, having a value of $1 million or more, or involving lobbyist services.

The forms must be electronically submitted to the Texas Ethics Commission before contract execution, with specific filing requirements and potential penalties for non-compliance. For the Certificate of Interested Parties, businesses must create an online account, submit the electronic form, obtain a certificate number, and provide a signed physical copy to the City. The Conflict of Interest Questionnaire must be filed within 7 business days of becoming aware of potential conflicts, and both forms should be submitted to the City Secretary's Office in Lubbock, Texas. Failure to comply can result in a Class C misdemeanor.
