EXECUTIVE SUMMARY
CTA Construction & Environmental, LLC. (CTA), on behalf of Pinyon Environmental, Inc., was retained by General Services Administration (GSA) to perform asbestos re-inspection services at the Mike Mansfield Federal Building/U.S. Courthouse (GSA Building number MT0004ZZ), located at 400 North Main Street in Butte, MT 59701-8866 (Site).
The building was constructed in 1904 and 1934. The facility inspected is a three-story, brick and stone structure with a full basement and an attic. It encompasses a total of 73,339 square feet of building space.
During this re-inspection completed on January 7 and 8, 2014, two additional suspect asbestos-containing building materials were identified. The materials were analyzed by Polarized Light Microscopy (PLM). Upon PLM analysis, the materials were determined to not contain asbestos.
Abatement History
Some of the ACBM(s) outlined in the 2007 inspection are no longer present in all, or any, of the locations detailed in that report. This information is summarized below, to extents feasible.
According to the 2007 inspection and supported by CTA’s field verification, all straight-run, Duplex® insulated pipes are domestic pipes. There are two (2) distinct Duplex® insulations present. The larger main city water supply lines exhibit a black core material underlying the felt layers (T3.5B), while the smaller domestic feeder lines have a white core material underlying the felt layers (T3.5A). This is true for both 1904 and 1934 construction dates. Results of additional sampling completed during the 2007 inspection indicated that only the white core, inner layer portion of the duplex insulation on domestic feeder lines (T3.5A) was asbestos-containing. The felt layers from both Duplex insulations and the black core material observed on T3.5B were determined to be non-asbestos.
In 2010, Abatement Contractors of Montana (ACM) were contracted to remove and clean asbestos-containing pipe insulation and associated debris from the west side of the basement of the facility. According to the clearance report by Affinity Environmental, dated November 18, 2010, the aforementioned materials were abated from the following Rooms: B-13; B-17; B-19; B-21; and, B-23. This abatement/clearance report has been included for reference as Appendix E. CTA observed during this re-inspection, that similar materials were either completely and/or partially removed from the following additional Rooms: B-03; B-03B; B-09; B- 11; B-15; and, B-25, whether by ACM or another contractor. Additionally, CTA observed that boiler flue insulation materials (T6.2) had been removed completely from Room B-25 and partially from Room B-05, and that the cut edge was sealed with spray foam to limit any potential material deterioration and/or exposures.
In 2013, ACM was contracted to remove and clean asbestos-containing pipe insulation and associated debris from the attic space of the facility. According to the clearance report by Indoor/Outdoor Environmental, LLC, ACM removed the aforementioned materials from approximately 150 foot of piping, as documented by CTA observation along the north end of Rooms 401 and 402 and from the east side of Room 402. This abatement/clearance report has been included for reference as Appendix E. Two (2) areas of damaged Thermal System Insulation (TSI) were observed by CTA along Room 401. CTA used a spray adhesive to coat over the damaged areas observed during this re-inspection to further limit the potential for any fiber release and/or exposures.
Two (2) ACBMs outlined in the 2007 inspection report were not observed by CTA personnel during the 2014 re-inspection. These materials include the following: olive/gray 9-inch by 9-inch floor tiles with orange and white streaks (F3.1-04); and, 2-foot by 5-foot ceiling tiles with lengthwise deep large fissures and pinholes (M5.5-34).
Asbestos Findings

O&M Operations and Maintenance
1. Introduction and Scope of Work
CTA Construction & Environmental, LLC. (CTA) was retained by Pinyon Environmental who was retained by General Services Administration (GSA) to perform a re-inspection asbestos survey of the Mike Mansfield Federal Building/U.S. Courthouse (GSA Building number MT0004ZZ), located at 400 North Main Street in Butte, MT 59701-8866. The purpose of this survey was to identify friable and non-friable suspect asbestos-containing building materials (ACBM).
The scope of this inspection included the following activities:
Surveying, identifying, and sampling suspect friable and non-friable ACBM.
Preparing this comprehensive report documenting the sampling procedures and results of the ACBM survey.
To accomplish this survey, all accessible areas of the building were identified by name and assigned room numbers.
A summary of identified functional spaces is presented in Table 3-1.
Within each space, suspect building materials were identified. C T A uses a standardized numbering format to identify suspect building materials, and to identify bulk samples.
The report contains six sections. The six sections are: introduction and scope of work; inspection procedures;
survey findings; conclusions and recommendations; removal cost estimate; and limitations sections. The report also contains seven appendices. Appendix A contains figures, Appendix B contains tables, Appendix C contains the laboratory analysis reports and chain of custodies, Appendix D contains a Photographic Log, Appendix E contains abatement and/or clearance report documents, Appendix F contains general inspection information, and Appendix G contains inspector and laboratory certification.
2. Inspection Procedures
The asbestos survey was performed using the applicable portions of the currently recognized standard protocol developed for schools under AHERA, as promulgated in Title 40, Code of Federal Regulations (40 CFR), Part 763 and as amended in the Federal Register (EPA, 2011). Since the primary concern for this investigation was to identify potential asbestos hazards in the entire building, a CTA representative visually inspected existing conditions within the entire facility. An overview of asbestos and current regulations and a glossary of common terms are presented in Appendix F.
2.1 Inspection and Sampling
The inspection was conducted by our State certified and AHERA accredited asbestos inspector, and consisted of a detailed visual survey of surfacing materials, thermal system insulation, and miscellaneous materials throughout the building. Suspect ACBM was then grouped into homogeneous materials and sampling plans developed. Components of the inspection included:
Identification of homogeneous suspect materials on a room by room basis (Appendices A and B).
An assessment of known or assumed ACBM, generally classifying the materials using categories defined in the AHERA and National Emission Standards of Hazardous Air Pollutants (NESHAP), for asbestos. A description of NESHAP categories is presented in the Section 3 Room-By-Room Table Key.
Homogeneous suspect ACBM’s were, for the purpose of this study, placed into the following four materials type categories: Thermal System Insulation (TSI), surfacing materials, miscellaneous materials, and floor covering. AHERA sampling protocol specifies sampling procedures for each material type (Appendix F).
Sample locations for this survey were chosen in a non-random fashion, with emphasis placed on obtaining samples of each type of accessible, suspect material and minimizing damage to the material being sampled (Appendix A Figure A-1.1, A-1.2, A-1.3, A-1.4, A-1.5, A-1.6, and A-1.7). Samples were collected by carefully removing small portions of the suspect material in a non-abrasive manner. If possible, samples from existing damaged areas or loose pieces of material were collected. Samples were obtained by using techniques such as wet slicing, wet boring or similar methods designed to limit contamination of the area during sampling. The sampled area was sealed using patching compounds, duct tape, or spray encapsulates as appropriate to the material being sampled.
Immediately after collection, samples were placed in pre-labeled plastic containers. Containers with samples were then placed in a large re-sealable plastic bag for transportation to the laboratory.
Documentation of our inspector’s asbestos accreditations is included as Appendix G.
2.2 Laboratory Analysis
Bulk samples obtained during the inspection were assigned bulk sample numbers and entered on sample chain-of-custody forms, followed by being shipped to EMSL Analytical, Inc. (EMSL) in Cinnaminson, New Jersey.
Laboratory analysis was performed in accordance with EPA Method 600/R-93/116, which employs polarized light microscopy (PLM) with dispersion staining for identification of mineral forms of asbestos, and has a limit of detection (LOD) of approximately 1%, by volume.
2.3 Quality Assurance and Quality Control
Quality Assurance and Quality Control (QA/QC) measures adopted by CTA involved both field and office components. Key parameters are summarized below:
Field QA/QC
Review inspection forms for completeness
Check homogeneous materials listing for sufficient number of collected samples
Verify locations of major mechanical components
Office QA/QC
Review lab results for completeness
Ensure appropriate cross-referencing of results, forms for given ACBM
Ensure drawings are updated as necessary following field QC
Verify approximate quantities of ACBM based on drawing review
Review recorded field comments for meaning, incorporate as necessary into report
3. Survey Findings

Hazard Rank and Removal Priority
Per AHERA regulations, each known and/or assumed friable asbestos-containing material is assessed for condition and is assigned a Hazard Rank number and Removal Priority number which are listed in these two columns. The Hazard Rank and Removal Priority for Friable ACBM table is located on the following page as a supplement to Table B and details the procedure for assigning AHERA Hazard Ranking and Removal Priorities for materials based on the degree of damage observed.
Supplement to Table B: Room by Room Summary of Suspect and Confirmed ACBMs
