About this file

This is a Contract Modification #2 and Extension #1 document between the Franklin County Board of Commissioners and Metropolitan Life Insurance Company for the provision of voluntary group short and long-term disability insurance, group legal services plan benefits, and additional employee benefit offerings to members of the Franklin County Cooperative Health Improvement Program. The original contract was executed on January 18, 2022, under RFP #2020-06-35, with a First Amendment added on December 18, 2023, to include group legal services. This extension modifies and extends the contract for an additional 24-month term from January 1, 2025, through December 31, 2026, with no further renewal periods remaining. The modification adds pet insurance as an entirely employee-paid benefit available to all benefits-eligible employees, establishes optional Identity Theft insurance coverage at specified rates, and adjusts the group legal services plan participation fees.

The total not-to-exceed amount for this extension is $185,000.00. Renewal rates for voluntary long-term disability coverage reflect a 1.1 percent decrease across all age bands, with annual premiums ranging from $29,483 to $259,327 depending on age cohort and covered volume. Voluntary short-term disability rates decreased 4.2 percent, with annual premiums ranging from $60,605 to $276,662. Employer-paid short-term disability alternative plan options are available at rates between $0.136 and $0.238 per $10 covered weekly benefit. The employee-paid group legal services participation fee remains $16.50 per month, while a new employer-paid option is established at $8.25 per month. Identity Theft insurance is offered at rates from $3.00 to $12.95 per employee per month depending on plan tier and whether employer or employee funded. Metropolitan Life Insurance Company's notice contact has been updated to Charlie Cross, Senior Account Executive, located in Southfield, Michigan.
