About this file

This is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division, Department of Administration, applicable to the Nevada Department of Corrections' Boiler Preventative Maintenance and Service contract opportunity. The certification form serves as a mandatory disclosure document for vendors bidding on this state contract, which involves providing preventative maintenance and service for boilers at multiple Department of Corrections facilities throughout Nevada. The form must be completed and submitted by authorized representatives of bidding vendors as a prerequisite for contract award consideration.

The certification requires vendors to attest that no federally appropriated funds will be used to influence government officials in connection with the contract award process. Vendors must also commit to disclosing any non-federal funds used for lobbying activities via Standard Form-LLL if applicable, and to include identical certification language in all subcontracts and subawards. Failure to submit the required certification subjects vendors to civil penalties ranging from $10,000 to $100,000 per violation. The form requires completion of fields including the solicitation number, vendor name, authorized signer name, signature, and date of submission.
