About this file
This file is an appendix containing the IRS Form W-9 (Request for Taxpayer Identification Number and Certification) and supporting compliance documentation for a City Colleges of Chicago (CCC) contract opportunity for athletic training services. CCC is soliciting proposals from qualified vendors to provide six certified athletic training services supporting intercollegiate athletics programs across six colleges. The athletics program currently serves over 400 student-athletes competing on more than 25 athletic teams throughout the academic year. The file includes the standard W-9 form along with comprehensive terms and conditions addressing minority business enterprise (MBE) and women business enterprise (WBE) participation requirements, participation goals, compliance procedures, reporting obligations, and non-compliance remedies. Additionally, the documentation encompasses ethics orientation requirements for contractors and vendors, fiduciary responsibility policies, restrictions on use of district property, and political activity guidelines.
The submission requirements mandate completion of the W-9 form along with schedules addressing MBE/WBE goal implementation plans, certifications, joint venture agreements, and waiver requests where applicable. All vendor and contract workers are required to sign an ethics acknowledgment and submit an economic disclosure statement and affidavit. The file does not specify pricing terms, contract duration, renewal options, response deadlines, or award dates. Specific project details regarding service delivery locations, performance timelines, or funding amounts are not detailed in this appendix, indicating that such information would be contained in the primary contract solicitation documents.
