About this file
This is a Debarment and Suspension Certification document issued by the Cherokee County School District (CCSD) Procurement Services Department in Georgia for an LED Lighting Conversion RFP. The certification establishes that CCSD will solicit bids from and award contracts only to responsible contractors and subcontractors who are not debarred, suspended, ineligible, or excluded from contract awards by the U.S. Department of Agriculture or any other Federal, State, or Local Agency. Bidders must certify under penalty of perjury that neither they nor any subcontractors they may utilize are subject to debarment or suspension. The CCSD reserves the right to award projects to multiple proposers, with specific location assignments to be determined by CCSD upon award. All questions regarding the RFP must be submitted through the Euna Procurement Message Center.
Bidders are required to execute and sign the certification as a condition of contract eligibility; failure to complete this certification will result in disqualification from award consideration. Additionally, bidders must immediately notify CCSD if they become debarred or placed on the Consolidated List of Debarred, Suspended, and Ineligible Contractors by any Federal entity at any point during the bid process or after contract award. The document requires certification signature by a corporate officer with authority to bind the company and includes fields for the legal name of the bidder, corporate officer signature, title, and printed name. This certification was last updated in August 2023.
