About this file
This is an Elevator Modernization Vendor Questionnaire designed to collect information from contractors seeking to perform elevator modernization services at VA Medical Centers. The questionnaire gathers basic company information including company name, point of contact, and Unique Entity Identifier (UEI), along with financial capacity metrics such as single project bonding capacity, aggregate bonding capacity, and surety company details.
The document requires vendors to confirm compliance with specific insurance requirements: workers' compensation and employers liability coverage (minimum $100,000), general liability insurance of $500,000 per occurrence without asbestos exclusionary clauses, automobile liability of $200,000 per person/$500,000 per occurrence with $20,000 property damage coverage. Additionally, vendors must disclose the number of certified elevator mechanics employed as of January 15, 2026; concurrent modernization projects completed within a one-year period; all office and branch locations; geographic service areas by state; capacity to perform simultaneous projects at multiple VA Medical Centers with the number of concurrent projects they can handle; and current Experience Modification Rate (EMR) from their insurance carrier with written explanation if the EMR exceeds 1.0, including the anticipated date for reduction to 1.0 or below. The questionnaire includes a comprehensive list of all 50 states for vendors to indicate where they currently perform elevator modernizations.
