Can facility define what areas will be required to work specific hours, days, times, etc.? It was stated during job walk that certain areas would be restricted to specific days and times to accommodate hospital patients and staff, please clarify

The OR and SPS are generally shut down for 2 weeks annually, usually in August. Work for those areas will likely be restricted to this 2 week period. No more than 2 rooms per inpatient ward can be taken simultaneously, with work in the inpatient wards occurring during the day.

Admin space can be done off‐hours when inpatient ward work is not available. Mental health facilities will need to be specifically coordinated as well, but will likely be daytime work.

TAB Scope Clarification

Project: VA PHX EHRM Project No: 00961.00

Date of Issue: 06/25/2021

Purpose: To better define the scope of work regarding the Test and Air Balance portion of this project.

All additions, corrections and clarifications are to be submitted to DHE Engineering within five days of receipt.

1. Pre-Construction: Anywhere a new Terminal Unit is being added or removed, rebalancing will need to take place. Prior to demolition there will be multiple airflow readings that need to be taken. First, perform a traverse reading in the duct mains upstream of where the work will take place. Next, perform airflow measurements on all diffusers and grilles downstream of where the traverse measurement was taken.

Once these measurements have been taken, submit to A/E team for their records.

2. Post-Construction: After construction is complete rebalancing will need to take place to ensure the spaces around the renovated rooms are left as they were prior to construction. Perform another traverse airflow measurement at the same spot as the traverse taken during pre-construction. Balance the system or adjust airflow from the Air Handler as needed to match the airflow that was acquired during the pre construction measurement. The same will need to be performed for all the diffusers and grilles that had pre construction airflow readings. Verify that the post construction measurement for all diffusers and grilles matches the pre construction measurements, adjust system as needed to acquire the correct airflow readings.

Scope

Construction Hospital operations shall not be unexpectedly impacted during this project. All impacted shall be planned with the COR as an outage (See more information in part 11 below). Patient care cannot be compromised during this project; therefore, it is highly likely construction will require carefully coordinated phasing to not impact patient care. Minor outages may be permitted if no reasonable alternatives exist but must be planned and approved in advance of the requested date. See below for more details.

Successful outcomes include, but not limited to, the following requirements:

NOTE: Requirements are the roadmap to the outcomes.

Requirements begin with the CFM Program Guide for Construction and reference to the design Technical Information Library (TIL).

1) Work Hours – Normal work hours are Monday through Friday, 0630-1630. Times for weekend work and evening work, if required, will be agreed upon by the contractor and

COR.

2) Submittals – Contractor shall use Submittal Exchange (or equal) as a means of communicating with the VA. Submittal Exchange shall be provided within 72 hours after award.

3) Daily Reports – Contractor shall provide a daily report. The daily report shall include any safety issues, personnel onsite and the company they work for along with the hours worked.

a. The daily reports shall be provided to the COR within 24 hours of each workday.

b. The safety inspection form shall have the project number, project start date, project end date and an area for the date and initials by the contractor’s safety officer.

c. The safety inspection form shall be filled out by the contractor’s safety officer at the beginning and end of each workday.

d. The Contractor shall comply with ICRA and ILSM requirements during construction.

4) Schedule of Values (SOV) – Contractor’s Schedule of Values shall be submitted within 48 hours after award of the contract.

a. The SOV will be broken down by phase of construction and CSI division of work and materials.

b. The cost of the General Conditions per day must also be presented on contractors the Schedule of Values.

c. The Schedule of Values is to be presented as a submittal for the VA COR review and Contracting Officer’s written approval.

d. Contractor to provide a detailed cost schedule for each section within the specification’s table of contents, provided in MS Excel format.

5) Outages – The scheduled timing of all outages requires a 21 calendar days’ written notice to the VA’s Contracting Officer’s Representative (COR). This overrides the 7 and 15 day outage timeframes in the specifications.

a. All work closing down any section of the hospital shall be considered an outage.

b. Any work impacting the telecommunications of part of the facility shall be considered an outage.

c. Written notice shall include:

i. Details of the work being performed including tasks and personnel performing the work.

ii. All changes to the ILSM and ICRA. Work with the COR to identify these.

iii. Duration of the outage, times, and dates.

d. All outages shall be approved in writing by the COR 14 days’ in advance.

e. The priority for performing any outages are as follows:

i. Weekends between 0630 and 1500

ii. After 1630 on weekdays

iii. After 1500 on weekends

iv. During normal work hours

f. The contractor cannot perform the outage without written approval by the COR.

6) Vacating the Area – Before mobilization the VA will clear all movable items and equipment they will salvage.

a. The remaining serviceable and usable items will be removed by the contractor.

The VA reserves the right to claim any serviceable/usable items. Items not claimed by the VA will be the responsibility of the contractor to remove from the facility campus.

7) Pre-Construction Meeting – The pre-construction meeting shall be held before mobilization between, the contractor’s management, contractor’s site superintendent, contractor’s safety officer, VA contracting officer and VA contracting officers’ representative.

a. All training of clinician and maintenance personnel required shall be identified in writing by the contractor, prior the pre-construction meeting.

b. Identification is defined as listing the specification section, training requirements, personnel (company) performing the training and including the training in the overall project schedule.

c. All warranty items shall be identified in writing by the contractor no later than 50% of the project schedules completion.

d. A complete testing plan shall be provided at the pre-construction meeting.

i. Plan will list out all testing in the specifications and approximate percentage of project completion for when the test shall take place.

e. At the pre-construction meeting, the contractor shall submit to the VA, the critical path for the project, validating their review of the guide listed above.

8) Inspecting Personnel – Along with personnel listed in the requirements, during installation, the following items will be inspected by the personnel below and the COR before being covered/sealed:

a) Electrical work –Electrical Shop Supervisor

b) Mechanical work –HVAC Shop Supervisor

c) Plumbing work - Plumbing Shop Supervisor

d) Structural work – COR/AE

e) Data/Telecom/Special systems work –Special systems supervisor

f) IT Work – IT Supervisor

g) Miscellaneous work – COR/AE

9) Written Deliverables – It is the contractor’s responsibility to provide all written requirements on time. It is the contractor’s responsibility to request all written requirements from the COR on time.

10) Oversight – The Contractor shall always have a site superintendent present when there is work in progress and/or contractors onsite, including subcontractors.

a. COR shall be notified work will take place prior to any work commencing. This includes work of the contractor and all subcontractors.

11) Critical Path – The following areas will have limited access and are considered critical path of this project. Administration space shall be the alternate area to perform work when the contractor cannot work in the below space:

a. ICU

b. Inpatient Wards to include 2C, 4C, 4D, 3B, 2A

c. Mental Health Wards 5C and 5D

d. Inpatient Radiology

e. Surgery

f. Jade Opal Clinic

12) Work to be performed – This project includes communications infrastructure upgrades within one primary facility and eleven other minor buildings across the Phoenix Veterans Administration Medical Center Campus. This includes either the relocation/replacement or remodel of approximately 47 existing telecommunications rooms (TRs) and one data center, to bring them up to VA Electronic Health Records Modernization (EHRM) IT standards while IT services remain operational. The contractor shall provide a turn-key project for these rooms in accordance with VA Office of Information and Technology (OIT) and EHRM infrastructure standards, as follows:

a. Construct two new towers along the exterior of the existing primary facility.

Towers are constructed using structural steel HSS sections, with light steel framing wall infill, and potential to support off-site fabrication in modules. The towers will contain in total, ten TR’s, ten small electrical rooms and ten small mechanical rooms. New TR’s in towers will replace approximately fifteen existing TR’s.

b. Expand three existing TR’s in the primary facility and one TR in a minor building on campus.

c. Renovate approximately twenty-eight TR’s by upgrading HVAC, electrical and telecom infrastructure.

d. Installation of upgraded communication infrastructure (new data outlets, patch panels, upgrade to Cat 6A cable as needed to achieve 1GB connectivity to end user devices. Upgrade and provide redundant fiber-optic cabling.

e. Installation of all low voltage raceway systems and room fittings to support network equipment, including all required bracing/backing/mounting materials and back boxes to support installation of IT equipment.

f. Installation of required power, grounding, uninterruptible power supplies, building management system, fire protection/alarm and security system interfaces.

Address HVAC requirements, and other system requirements, for TRs/data centers in the area or associated with the area of work.

g. Removal of all legacy IT equipment and room fittings (e.g., racks, cable trays) from any TR that is being abandoned due to TR relocations under the scope of this effort. Removal of abandoned or replaced cabling.

NOISE AND VIBRATION CONTROL FOR HVAC PIPING AND EQUIPMENT

PART 1 - GENERAL

1.1 DESCRIPTION

A. This section specifies the application of noise control measures, and vibration control techniques to boiler plant rotating equipment and parts including chillers, cooling towers, boilers, pumps, fans, compressors, motors and steam turbines.

B. A complete listing of all common acronyms and abbreviations are included in Section
