Item 1A Service Description Inspecting and testing QTY 350 Fire Dampers Amount $

Item 1B Service Description Related Tasks and Deliverable Amount $

Additional existing fire dampers identified during inspection to be added to inventory and floor plans.

a) 50 Quantity NTE Unit Each Unit Price $_______

b) Verification of damper location.

c) Labeling damper location and ID# on ceiling grid and life safety drawings.

d) Tag damper using label identifying damper ID, inspection date and pass or fail rating.

e) Determination of access to damper access door, damper blades and external components.

f) Picture of damper in its current state “Before”.

g) Clean each damper of any debris that would impede normal operation.

h) Lubricate damper track.

i) Removal of fusible link.

j) Picture of damper in its closed state “After”.

k) If the fusible link is damaged, painted or the incorrect rating it shall be replaced with a new fusible link.

l) If the damper is a smoke and combination type damper the power source shall be removed from its actuator to verify damper closure.

m) If the testing of the damper fails minor repairs are to be attempted to correct. If minor repairs can’t be made a recommendation to replace the damper (cost not included in this project) will be submitted on a separate proposal.

n) If damper is deemed inaccessible a best solutions recommendation (cost not included in this project) will be submitted on a separate proposal.

o) Damper assessment spreadsheet detailing damper ID#, location, ductwork size, damper type, fusible link rating, pass/fail status, correction recommendation, and comments.

p) If the contractor discovers additional access issues they shall immediately be brought to the attention of the COR for resolution

b) Install sheetrock ceiling access panel for proper access to damper.

c) Install fire rated sheetrock access panel in fire rated wall where needed for access to damper.

d) Inspect damper, Install RED Tag and Document.
