1.3 Nature and Sequence of Construction Activity

Describe the general scope of the work for the project, major phases of construction, etc:

The purpose of this project is to enable an existing national cemetery to continue to provide burial services for eligible veterans. In accordance with the master plan for the entire 640-acre site, this project will develop approximately 30 acres of undeveloped land to provide approximately 19,000 gravesites, including both casket and cremation sites in new burial sections and redevelopment to more efficient interment methods in existing sections. This project will provide for an additional 10-year inventory of pre-placed crypts for casketed interments and both in-ground sites and columbarium niches for cremated inurnments as well as various cemetery improvements. Although the cemetery has an existing inventory of traditional casket gravesites beyond the year 2030, the existing inventory of the more efficient pre-placed crypts are projected to deplete in December 2023. This project will convert the less efficient traditional casket gravesites to the more efficient pre-placed crypts to provide for an additional 10-year inventory. Additionally, the NCA statistical data indicates columbarium niches for cremated inurnments are available until September 2024 and a depletion of in-ground cremation gravesites by December 2027. Included in this project is the conversion of traditional casket sites to pre-placed crypt sites. Only those facilities and infrastructure elements necessary to support the added maintenance, operations, and burial capacity for approximately 10 years will comprise this phase of construction. The design and documentation of this project will be accomplished following the National Cemetery Administration (NCA) Facilities Design Guide and the VA program Guide (PG-18-15, Volume D), A/E Submission Instructions for National Cemetery

Projects. The design will culminate in the preparation of construction drawings and specifications to be used for competitive bidding.

What is the function of the construction activity?

― Briefly describe the nature of the construction activity and approximate time frames (one or more paragraphs, depending on the nature and complexity of the project).

― For more information, see SWPPP Guide, Chapter 3.A.

Estimated Project Start Date: 05/18/2019

Estimated Project Completion Date: 02/05/2021

1.4 Soils, Slopes, Vegetation, and Current Drainage Patterns

Soil type(s):

The National Resources Conservation Service (NRCS) Site Soil Mapping shows the site to be

40.4% Ferris-Heiden complex, 29.6% Heiden clay, 9.8% Houston Black clay, 0.7% Lewisville silty clay, 2.6% Ovan clay, 13.9% Vertel clay (and 3% water area in AOI). These soils are generally identified as being Hydrologic Soil Group C/D.

Slopes (describe current slopes and note any changes due to grading or fill activities):

The site is currently very hilly with hills with slopes of 10-18% leading to low creeks running through the site. The site generally slopes in the southeasterly direction towards an existing creek

(Mountain Creek) which then flows north and west to Mountain Creek Lake, directly to the north of the site.

Drainage Patterns (describe current drainage patterns and note any changes dues to grading or fill activities):

There is one minor drainageways across this site. Mountain Creek flows southward along the eastern portion of the site, then turns west across the southern portion of the site, and then northwest to Mountain Creek Lake, directly north of the site. All runoff from the site drains to

Mountain Creek. Proposed site conditions will not change the general direction of flows into

Mountain Creek. Water generally flows south and east overland, into the creek.

Vegetation:

Natural vegetation at the existing site is thick underbrush and tall grasses on the south and eastern edges of the site, near Mountain Creek. Generally, across the site, grasses and small shrubs are dominant.

Instructions:

― Describe the existing soil conditions at the construction site including soil types, slopes and slope lengths, drainage patterns, and other topographic features that might affect erosion and sediment control.

― Also, note any historic site contamination evident from existing site features and known past usage of the site.

― This information should also be included on your site maps (See SWPPP Guide, Chapter 3.C.).

Other:

1.5 Construction Site Estimates

The following are estimates of the construction site.

Total project area: 120.1 acres

Construction site area to be disturbed : 50.71 acres

Percentage impervious area before construction: 10 %

Runoff coefficient before construction: 0.4

Percentage impervious area after construction: 20%

Runoff coefficient after construction 0.4

Instructions:

― Estimate the area to be disturbed by excavation, grading, or other construction activities, including dedicated off-site borrow and fill areas.

― Calculate the percentage of impervious surface area before and after construction

― Calculate the runoff coefficients before and after construction.

― For more information, see SWPPP Guide, Chapter 3.A and Appendix C.

1.6 Receiving Waters

Description of receiving waters: The main receiving body of water that accepts runoff flows from this site is Mountain Creek, which then flows into Mountain Creek Lake, directly to the north of the project site.

Description of storm sewer systems: Storm water flows shall be conveyed through curb, gutter, and inlets to small tailwater basins or small water quality ponds before being released directly into existing channels that lead to Mountain Creek (or directly into the creek itself).

Description of impaired waters or waters subject to TMDLs: N/A

SECTION 5: INSPECTIONS

5.1 Inspections

1. Inspection Personnel: Identify the person(s) who will be responsible for conducting inspections and describe their qualifications:

A qualified person knowledgeable in the principles and practice of erosion and sediment controls and pollution prevention, and who possesses the skills to assess construction site conditions that could impact storm water quality, and the skills to assess the effectiveness of any storm water controls, is responsible for conducting regular inspections. Table 5-1 will identify personnel responsible for performing inspections prior to the start of construction, and a copy will be kept onsite.

At a minimum, a site inspection must be conducted once every 14 calendar days and within 24 hours of the occurrence of a storm event of 0.25 inches or greater. To determine if a storm event of

0.25 inches or greater has occurred on the Project site, a properly maintained rain gauge must be maintained on site, or the storm event information from a weather station that is representative of the Project location must be obtained. For any day of rainfall during normal business hours that measures 0.25 inches or greater, the total rainfall measured for that day must be recorded.

Table 5-1: Storm Water Inspector Identification List

Personnel Title Contact Responsibility

― Identify the individual(s) responsible for conducting inspections and describe their qualifications.

Reference or attach the inspection form that will be used.

― Describe the frequency that inspections will occur at your site including any correlations to storm frequency and intensity.

― Note that inspection details for particular BMPs should be included in Sections 2 and 3.

― You should also document the repairs and maintenance that you undertake as a result of your inspections.

These actions can be documented in the corrective action log described in Part 5.3 below.

― For more on this topic, see SWPPP Guide, Chapters 6 and 8.

― Also, see suggested inspection form in Appendix B of the SWPPP Guide.

2. Inspection Schedule and Procedures:

Describe the inspection schedules and procedures you have developed for your site (include frequency of inspections for each BMP or group of BMPs, indicate when you will inspect, e.g., before/during/and after rain events, spot inspections):

During site inspections, the inspector must complete the following actions:

• Check whether all erosion and sediment controls and pollution prevention controls are installed and operational, and determine if controls require replacement, repair, or maintenance

• Check for the presence of conditions that could lead to spills, leaks, or other accumulations of pollutants

• Identify locations where new or modified storm water controls are necessary to meet the requirements of Parts 2 and/or 3 of the Permit

• Check discharge points and banks of any surface waters flowing within the Project boundaries or adjacent areas for signs of visible erosion and sedimentation that are attributable to site discharge;

if discharge points are not accessible, check nearby downstream locations to the extent practicable

• Identify incidents of noncompliance

• If discharge is occurring at the time of an inspection:

• Identify all points of the property from which a discharge occurs

• Observe and document the visual quality of the discharge and take note of the characteristics of the storm water discharge, including color, odor, floating, settled, or suspended solids, foam, oil sheen, and other obvious indicators of storm water pollutants

• Document whether storm water controls are operating effectively and note controls that are not operating effectively

• Inspect locations where vehicles enter or exit the Project site for evidence of offsite sediment tracking along county or State paved roadways

In areas of the site where construction activities have permanently or temporarily ceased and will not resume for a period exceeding 14 calendar days, stabilization measures must be initiated. In areas where seeding has been completed, appropriate non-vegetative cover (hydro mulch or erosion control blankets) has been provided, or temporary, non-vegetative stabilization (riprap, geotextiles, gabions etc.) has been installed, inspection frequency may be reduced to once per month.

Inspections shall immediately increase to the standard frequency if construction activities resume.

Based on the results of the inspection, initiate corrective action.

Describe the general procedures for correcting problems when they are identified. Include responsible staff and time frames for making corrections:

Attach a copy of the inspection report you will use for your site. – See Appendix E
