About this file

This is a scoring sheet for a Request for Proposals (RFP) issued by Salt Lake City Corporation for claims adjustment services under solicitation number SLCI26001. The evaluation process consists of four stages: Stage 1 establishes minimum mandatory requirements including certification of ethics compliance, identification of conflicts of interest, verification of five years of relevant experience, and confirmation of an office location in Salt Lake County, Utah with availability to accept claim assignments Monday through Friday from 8 a.m. to 5 p.m. Stage 2 comprises scoreable technical criteria weighted across six categories—qualifications and proposed staff (20 points), approach to scope of work (30 points), litigation experience (25 points), approach to increase requests and fees (30 points), software and data privacy protections (20 points), and investigation and claims management processes (35 points)—totaling 800 possible points. Stage 2 also includes policy criteria worth 40 additional points across sustainability, disadvantaged business enterprise participation, apprenticeships, and local small business considerations. Proposals must achieve a minimum of 588 points (70 percent) in the technical evaluation to proceed to Stage 3.

Stage 3 of the evaluation process includes interviews weighted at 36 points with a required minimum threshold of 108 points (60 percent) to remain competitive. Stage 4 addresses cost evaluation, comprising 24 percent of the total evaluation score with a maximum of 265.26 points allocated using a comparative pricing formula in which the lowest-cost offeror receives the full allocation and other offerors receive points proportional to their cost relative to the lowest bid. The total evaluation framework allocates 57 percent weight to technical evaluation (840 points), 19 percent to interviews (180 points), and 24 percent to cost (265.26 points), for a combined maximum of 1,285.3 evaluation points.
