About this file

This is a Debarment and Suspension Certification form required for the Laguna Creek Trail and Bruceville Road Sidewalk Improvements Project in California. The form mandates that bidders certify their eligibility to participate in federally-regulated contracting by affirming that they, along with any associated owners, partners, directors, officers, or managers, are not currently suspended, debarred, voluntarily excluded, or deemed ineligible by any Federal agency. Bidders must also confirm that they have not experienced such debarment or suspension within the past three years, do not have a proposed debarment pending, and have not been indicted, convicted, or subject to adverse civil judgment involving fraud or official misconduct within the same three-year period.

The certification operates under Title 49, Code of Federal Regulations, Part 29, and must be submitted with the bid as a binding component of the proposal. Bidders may note exceptions to the certification if applicable; however, such exceptions do not automatically disqualify a bid but will be evaluated as part of the bidder responsibility determination. False information provided on this certification may result in criminal prosecution or administrative sanctions. The form requires the contractor or subcontractor signature, date, and title, with the act of signing the proposal constituting signature of this certification.
