SUMMARY OF WORK

08/15

PART 1 GENERAL

1.1 WORK COVERED BY CONTRACT DOCUMENTS

1.1.1 Project Description

The project's scope of work includes the new construction of three new facilities and the selective demolition and renovations of one existing facility in the MSA2 area of Andersen AFB, Guam. The work also includes the removal and replacement of the roads serving the Munitions area, East Perimeter Road between connection to north end of the airfield to the taxiway adjacent to the MSA2 area. The work is located in a secured area of the base and requires the Contractor, and subcontractors, to obtain special clearance and attend airfield driving training classes to access these areas.

Work will include the construction of two cast-in-place concrete earth covered magazines (4,400 SF), a new Powered Trailer Maintenance Facility (PTMF) of cast-in-place concrete walls and roof structure (8,273 SF) and renovations/additions to the Missile Assembly Shop (14,168/ 16,288 SF).

The existing asphalt roadways will be demolished and replace with new reinforced concrete pavement (340,000 SF) capable of supporting maximum vehicle weight of 80,000 lbs. Work also includes sitework clearing and grubbing, grading and extension of existing utilities infrastructure to the new facilities.

1.1.2 Location

The work is located at the Andersen Air Force Base, Guam, approximately as indicated. The exact location will be shown by the Contracting Officer.

1.2 OCCUPANCY OF PREMISES

Building 51109 (MASM) will not be occupied during performance of work under this Contract. Occupancy notifications will be posted in a prominent location in the work area.

Before work is started, arrange with the Contracting Officer a sequence of procedure, means of access, space for storage of materials and equipment, and use of approaches, corridors, and stairways.

1.3 EXISTING WORK

In addition to FAR 52.236-9 Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements:

a. Remove or alter existing work in such a manner as to prevent injury or damage to any portions of the existing work which remain.

b. Repair or replace portions of existing work which have been altered during construction operations to match existing or adjoining work, as approved by the Contracting Officer. At the completion of operations, existing work must be in a condition equal to or better than that

Jacobs WFXO6312 SECTION 01 11 00 Page 1 which existed before new work started.

1.4 LOCATION OF UNDERGROUND UTILITIES

Obtain digging permits prior to start of excavation, and comply with Installation requirements for locating and marking underground utilities.

Verify existing utility locations indicated on contract drawings, within area of work.

Identify and mark all other utilities not managed and located by the local utility companies. Scan the construction site with Ground Penetrating Radar (GPR), electromagnetic, or sonic equipment, and mark the surface of the ground or paved surface where existing underground utilities are discovered. Verify the elevations of existing piping, utilities, and any type of underground or encased obstruction not indicated, or specified to be removed, that is indicated or discovered during scanning, in locations to be traversed by piping, ducts, and other work to be conducted or installed. Verify elevations before installing new work closer than nearest manhole or other structure at which an adjustment in grade can be made.

1.4.1 Notification Prior to Excavation

Notify the Contracting Officer at least 15 calendar days prior to starting excavation work.

1.5 NCTS/Telecomm/Access Control/IDS Coordination Requirements

1.5.1 NCTS/Telecomm/Access Control/IDS Contractor Access

Allow the NCTS/telecomm/access control/IDS Contractor access to the facility towards the end of construction (finishes 90 percent complete, rough-in 100 percent complete, Inside Plant (ISP)/Outside Plant (OSP) infrastructure in place) to provide equipment in the telecommunications rooms and make final connections. Coordinate efforts with the NCTS/telecomm/access control/IDS Contractor to facilitate joint use of building spaces during the final phases of construction. After the Contracting Officer has facilitated coordination meetings between the two contractors, within one week, incorporate the effort of additional coordination with the NCTS/telecomm/access control/IDS Contractor into the construction schedule to demonstrate a plan for maintaining the contract duration.

1.4 CONTRACTOR ACCESS AND USE OF PREMISES

1.4.1 Activity Regulations

Ensure that Contractor personnel employed on the Activity become familiar with and obey Activity regulations including safety, fire, traffic and security regulations. Keep within the limits of the work and avenues of ingress and egress. To minimize traffic congestion, delivery of materials must be outside of peak traffic hours (6:30 to 8:00 a.m. and 3:30 to 5:00 p.m.) unless otherwise approved by the Contracting Officer. Wear hard hats in designated areas. Do not enter any restricted areas unless required to do so and until cleared for such entry. Mark Contractor equipment for identification.

1.4.1.1 Subcontractors and Personnel Contacts

Provide a list of contact personnel of the Contractor and subcontractors including addresses and telephone numbers for use in the event of an emergency. As changes occur and additional information becomes available, correct and change the information contained in previous lists.

1.4.1.2 Contractor Requirements for Access to Andersen Air Force Base

a. All Contractors shall have a sponsor to the installation on Andersen Air Force Base (AFB). Coordinate access to Andersen Air Force Base with the Contracting Officer.

b. A Contractor Affidavit is required for each person requesting access to Andersen AFB. Complete the "USAF Contractor/Sub-Contractor Access Affidavit" form.

c. Contractors are required to submit a paper pass/Base Pass Request with approval from their sponsor/Contracting Officer to the Visitors control Center.

1.4.1.3 Installation Access

Obtain access to Air Force installations through participation in the Defense Biometrics Identification System (DBIDS). Requirements for Contractor employee registration, and transition for employees currently under Navy Commercial Access Control System (NCACS), are available at https://www.cnic.navy.mil/om/dbids.html . No fees are associated with obtaining a DBIDS credential.

Participation in the DBIDS is not mandatory, and Contractor personnel may apply for One-Day Passes at the Base Visitor Control Office to access an installation.

1.4.1.3.1 Registration for DBIDS

Registration for DBIDS is available at https://www.cnic.navy.mil/om/dbids.html . Procedure includes:

a. Present a letter or official award document from the Contracting

Jacobs WFXO6312 SECTION 01 14 00 Page 3

Officer, that provides the purpose for access, to the base Visitor Control Center representative.

b. Present valid identification, such as a passport or Real ID Act-compliant state driver's license.

c. Provide completed SECNAV FORM 5512/1 to the base Visitor Control Center representative to obtain a background check. This form is available for download at https://www.cnic.navy.mil/om/dbids.html .

d. Upon successful completion of the background check, the Government will complete the DBIDS enrollment process, which includes Contractor employee photo, finger prints, base restriction and several other assessments.

e. Upon successful completion of the enrollment process, the Contractor employee will be issued a DBIDS credential, and will be allowed to proceed to worksite.

1.4.1.3.2 DBIDS Eligibility Requirements

Throughout the year, the Contractor employee must continue to meet background screen standards. Periodic background screenings are conducted to verify continued DBIDS participation and installation access privileges. DBIDS access privileges will be immediately suspended or revoked if at any time a Contractor employee becomes ineligible.

An adjudication process may be initiated when a background screen failure results in disqualification from participation in the DBIDS, and Contractor employee does not agree with the reason for disqualification.

The Government is the final authority.

1.4.1.3.3 DBIDS Notification Requirements

a. Immediately report instances of lost or stolen badges to the Contracting Officer.

b. Immediately collect DBIDS credentials and notify the Contracting Officer in writing under the following circumstances:

(1) An employee has departed the company without having properly returned or surrendered their DBIDS credentials.

(2) There is a reasonable basis to conclude that an employee, or former employee, might pose a risk, compromise, or threat to the safety or security of the Installation or anyone therein.

1.4.1.3.4 One-Day Passes

Personnel applying for One-Day passes at the Base Visitor Control Office are subject to daily mandatory vehicle inspection, and will have limited access to the installation. The Government is not responsible for any cost or lost time associated with obtaining daily passes or added vehicle inspections incurred by non-participants in the DBIDS.

1.4.1.4 No Smoking Policy

Smoking is prohibited within and outside of all buildings on installation, except in designated smoking areas. This applies to

PART 1 GENERAL

1.1 SUBMITTALS

Government approval is required for submittals with a "G" designation;

submittals not having a "G" designation are for Contractor Quality Control approval. Submittals with an "S" are for inclusion in the Sustainability eNotebook, in conformance with Section 01 33 29 SUSTAINABILITY REPORTING.

Submit the following in accordance with Section 01 33 00 SUBMITTAL

PROCEDURES:

SD-01 Preconstruction Submittals

Earned Value Report; G

1.2 EARNED VALUE REPORT

1.2.1 Data Required

This contract requires the use of a cost-loaded Network Analysis Schedule (NAS). Schedule of Prices must not be used with cost-loaded Network Analysis Schedule (NAS). Use Earned Value Report derived from cost-loaded NAS. Provide a detailed breakdown of the contract price, giving quantities for each of the various kinds of work, unit prices and extended prices.Costs shall be summarized and totals provided for each construction category Within 15 calendar days of notice of award, prepare and deliver to the Contracting Officer an Earned Value Report (construction contract) as directed by the Contracting Officer.

1.2.2 Schedule Instructions

Payments will not be made until the Earned Value Report from the cost-loaded NAS has been submitted to and accepted by the Contracting Officer. Identify the cost for site work, and include incidental work to the 5 ft line. Identify costs for the building(s), and include work out to the 5 ft line. Work out to the 5 ft line shall include construction encompassed within a theoretical line 5 ft from the face of exterior walls and shall include attendant construction, such as pad mounted HVAC cooling equipment, cooling towers, and transformers placed beyond the 5 ft line.

1.2.3 Real Property Assets

Draft DD Form 1354, Transfer and Acceptance of Military Real Property filled in with the appropriate Real Property Unique Identifiers (RPUID) and related construction Category Codes to summarize the designed real property assets that apply to this contract. The Contractor shall meet with the Contracting Officer and the Real Property Accounting Officer during the Pre Construction Meeting and the Project Closeout Meetings to modify and include any necessary changes to the DD Form 1354.

The Contractor shall provide the Interim DD Form 1354 that uses the appropriate division of the RPUIDs/ Category Codes to represent the final constructed facility and include all associated cost. Coordinate the

Jacobs WFXO6312 SECTION 01 20 00.00 20 Page 1

Contractor's Price and Payment structure with the structure of the RPUIDs/ Category Codes. A blank editable DD Form 1354 in pdf format is available for download from the following web site:

http://www.dtic.mil/whs/directives/forms/eforms/dd1354.pdf

Divide detailed asset breakdown into the RPUIDs and related construction Category Codes and populate associated costs which represent all aspects of the work. Where assets diverge into multiple RPUID/ Category Codes, divide the asset and provide the proportion of the assets in each RPUID/ Category Code. Assets and related RPUID/ Category Codes may be modified by the Contracting Officer as necessary during course of the work.

Coordinate identification and proportion of these assets with the Government Real Property Accounting Officer.

Cost data accumulated under this section are required in the preparation of DD Form 1354.

1.2.4 Schedule Requirements for HVAC TAB

The field work Section 23 05 93 TESTING, ADJUSTING, AND BALANCING FOR HVAC shall be broken down in the Earned Value Report from the cost-loaded NAS by separate line items which reflect measurable deliverables. Specific payment percentages for each line item shall be determined on a case by case basis for each contract. The line items shall be as follows:

a. Approval of Design Review Report: The TABS Agency is required to conduct a review of the project plans and specifications to identify any feature, or the lack thereof, that would preclude successful testing and balancing of the project HVAC systems. The resulting findings shall be submitted to the Government to allow correction of the design. The progress payment shall be issued after review and approval of the report.

b. Approval of the pre-field engineering report: The TABS Agency submits a report which outlines the scope of field work. The report shall contain details of what systems will be tested, procedures to be used, sample report forms for reporting test results and a quality control checklist of work items that must be completed before TABS field work commences.

c. Season I field work: Incremental payments are issued as the TABS field work progresses. The TABS Agency mobilizes to the project site and executes the field work as outlined in the pre-field engineering report. The HVAC water and air systems are balanced and operational data shall be collected for one seasonal condition (either summer or winter depending on project timing).

d. Approval of Season I report: On completion of the Season I field work, the data is compiled into a report and submitted to the Government. The report is reviewed, and approved, after ensuring compliance with the pre-field engineering report scope of work.

e. Completion of Season I field QA check: Contract QC and Government representatives meet the TABS Agency at the jobsite to retest portions of the systems reported in the Season I report. The purpose of these tests are to validate the accuracy and completeness of the previously submitted Season I report.

Jacobs WFXO6312 SECTION 01 20 00.00 20 Page 2

f. Approval of Season II report: The TABS Agency completes all Season II field work, which is normally comprised mainly of taking heat transfer temperature readings, in the season opposite of that under which Season I performance data was compiled. This data shall be compiled into a report and submitted to the Government. On completion of submittal review to ensure compliance with the pre-field engineering report scope, progress payment is issued. Progress payment is less than that issued for the Season I report since most of the water and air balancing work effort is completed under Season I.

1.3 CONTRACT MODIFICATIONS

In conjunction with the Contract Clause DFARS 252.236-7000 Modification Proposals-Price Breakdown, and where actual ownership and operating costs of construction equipment cannot be determined from Contractor accounting records, equipment use rates shall be based upon the applicable provisions of the Guam Cost Guidebooks, Equipment Watch Blue Book or acceptable guidance specific to Guam to establish a set cost for equipment and equipment operations cost.
